Moving beyond individual choice in policies to reduce health inequalities: the integration of dynamic with individual explanations

Abstract: Current evidence points to the potential of systematically applying broader thinking about causal mechanisms, beyond individual choice and responsibility, to the design, implementation and evaluation of policies to reduce health inequalities. We provide a set of questions designed to enable critique of policy discussions and programmes to ensure that these wider mechanisms are considered.

“…People are able not just to make sense of the world in a general sense (Giddens, 1984 2-19) but also in ways which feed the very processes of active inference through which predictive processing takes place. In other words, we might think of the processes going on during predictive processing as both individual and as relational/social (Kriznik, Kinmonth, Ling, & Kelly, 2018). Typifications are an emergent property of the interactions between human agency and social structure and the interaction between social practices and the physical and biological environments that are ubiquitous to the human condition .…”

“…Additionally, future research is recommended to include social and environmental behavioural change determinants into analysis to extend our understanding beyond individuals as recommended by Kriznik et al (2018) and many other researchers.…”

“…A shift towards dynamic evaluation methods is needed to centre research attention on the proportion of the target audience who change (or not) and understanding which factors support positive change outcomes along with the factors contributing to undesired change. Responding to calls to direct efforts to understanding more complex relational and dynamic factors (Kriznik et al, 2018), this paper employs one dynamic analytical method drawing on available data from a social marketing food waste pilot program. Food waste is a global problem.…”

“…This remains a fundamental problem, especially for those working to apply the principles of EBM and public health. To highlight this, Kelly reiterated that knowing the cause (in a linear, regularities sense) of a disease does nothing to help understand how to prevent that disease . There is a traditional focus on “what” questions in EBM, and not enough on the “hows,” and this is a fatal flaw.…”
