“…We choose a sternotomy combined with an anterior thoracotomy, this was because of requirements during the operation. A similar access has been described for this problem before [9,10] and has been termed hemiclamshell approach; this proved successful in this case. The oscillating saw was helpful.…”

“…This incision was usually used for vascular injuries in mediastinal or anterolateral cervicothoracic junction and resection of tumors located in the cervicothoracic area or extended to the anterior mediastinum [2]. …”

“…In our case mucus retention and resistance to pulmonary rehabilitation might be considered as causes of pneumonia after discharge. The study of Lardinois et al revealed no significant difference in the incidence of complications after hemiclamshell incision compared with conventional incisions [2]. …”

“…This incision offers good exposure to the entire mediastinum and the chest cavity and is useful for severe chest trauma with massive hemorrhage requiring urgent access to the entire mediastinum and the chest cavity, large tumors of the anterior cervicothoracic junction, and lesions with the involvement of both the upper mediastinum and one lung. 8 In conclusion, an optimal approach is very important to remove mediastinal liposarcoma because of the diversity of size and location of lesions. The hemiclamshell incision may provide useful surgical access to achieve complete resection of the giant mediastinal mass, which extended from the anterior mediastinum in close relationship to adjacent structures to a chest cavity.…”
