Introduction:In this study, we aimed to determine the epidemiological properties of metallo-beta-lactamase-producing Pseudomonas aeruginosa (MBL-PA) isolates and to investigate the relationship between the presence of MBL-PA and patient morbidity and mortality. Materials and Methods: The study included carbapenem-resistant P. aeruginosa isolates recovered from various clinical specimens of 334 patients in Karadeniz Technical University Faculty of Medicine Hospital, a 900-bed university hospital in Trabzon, Turkey. MBL-related carbapenem-resistant PA strains were phenotypically investigated using the Modified Hodge test and the imipenem/imipenem-ethylene diamine tetra acetic acid combined disc tests. Multiplex polymerase chain reaction was used to investigate the presence of bla IMP , bla VIM , bla GIM , bla SIM , and bla SPM genes, which are responsible for MBL production. Clonal relationships among MBL-PA isolates were analyzed by pulsed-field gel electrophoresis. The patients' hospital records were retrospectively examined. Various demographic and clinical characteristics were evaluated in relation to MBL-PA. Results: Thirty-two (9.6%) of the carbapenem-resistant PA isolates were found to carry bla VIM and/or bla IMP , with three strains harboring both bla VIM and bla IMP . MBL-PA isolates were more resistant to aminoglycosides and quinolones. Eight Verona integron-encoded metallo-beta-lactamase-type MBL-PA isolates were found to be identical in adults, while several clonally-related clusters were observed among MBL-PA isolates in both the pediatric and adult inpatients. Compared to non-MBL carbapenem-resistant PA, the risk factors evaluated were found to have no association with MBL-PA. In addition, there was no statistically significant difference in mortality between patients from whom MBL-PA or non-MBL-PA was isolated. Conclusion: Although MBL-PA has been implicated in various healthcare-related outbreaks, no specific risk factor has been identified in association with MBL-PA isolation. To our knowledge, this is the first study in Turkey to detect P. aeruginosa isolates carrying both bla VIM and bla IMP .Giriş: Bu çalışmada metallo-beta-laktamaz üreten Pseudomonas aeruginosa (MBL-PA) suşlarının moleküler epidemiyolojik özelliklerinin belirlenmesi ve MBL-PA varlığının hasta mortalitesi ve morbiditesi ile ilişkisinin değerlendirilmesi amaçlandı. Pseudomonas aeruginosa is intrinsically resistant to several antimicrobial drugs classes. Various extended-spectrum beta lactamases (ESBL) types have been found in P. aeruginosa such as Pseudomonas extended resistance (PER) and Guiana extended-spectrum (GES) enzymes. The study aimed to evaluate the susceptibility of the ESBL producing P. aeruginosa strains that express blaGES and blaPER genes to commonly used antibiotics. A total of 28 P. aeruginosa clinical isolates was identified as ESBL producers and subjected to polymerase chain reaction (PCR) technique for detection of blaGES, blaPER genes. Routine antimicrobial susceptibility was determined by the disc diffusion method. The highest resistance rate reached 71.4% for ciprofloxacin, while the lowest resistance rate (10.7%) was seen in imipenem followed by colistin (21.6%). blaGES gene was observed in 78.6% of the isolates, while blaPER appeared in 22.4%. It was concluded that imipenem and colistin showed good antipesuedomonal activity and blaGES was predominant gene among the ESBL producing P. aeruginosa in Makah hospitals. The results of the present study can help to prevent the mortality and morbidity associated with Pseudomonas infections in hospitals.

Bu çalışmada nozokomiyal infeksiyon etkeni olarak izole edilen P. aeruginosa izolatlarında Metallo-Beta-Laktamaz (MBL) üretimi ile bu bakterilerin antibiyotiklere duyarlılıklarının belirlenmesi amaçlandı. Gereç ve Yöntem: Laboratuvarımızda izole edilen ve CDC kriterlerine göre hastane infeksiyonu etkeni olarak tanımlanan, imipenem dirençli 62 P. aeruginosa izolatı çalışmanın gerecini oluşturdu. MBL varlığını belirlemek için kombine disk testi uygulandı. Çalışmaya alınan imipenem dirençli P. aeruginosa izolatlarının antibiyotik duyarlılık testlerinde BD Phoenix otomatize mikrobiyoloji sisteminden (Becton Dickinson, ABD) yararlanıldı. Bulgular: Hastane infeksiyonu etkeni imipenem dirençli P. aeruginosa izolatlarının kombine disk testi ile 53 (%85)'ünde MBL varlığı belirlendi. İmipenem dirençli izolatların diğer antibiyotiklere direnç oranları; amikasin %10, siprofloksasin %53, seftazidim %65, levofloksasin %68, aztreonam %84, gentamisin %90, piperasilintazobaktam %92, sefepim %97, piperasilin %100 olarak bulundu. Sonuç: Nozokomiyal kökenli P. aeruginosa suşlarında MBL üretimi giderek artmaktadır. Dirençten sorumlu olan bu enzim, her laboratuvarda uygun bir fenotipik yöntemle rutin olarak saptanmalı ve rapor edilmelidir. Bu uygulama direnç yayılımının önlenmesinde ve infeksiyonların tedavisinde önemli yararlar sağlayacaktır.
