Schistocytes are fragmented red blood cells produced as a result of mechanical damage to erythrocytes, usually due to microangiopathic thrombotic diseases or mechanical factors. The early laboratory detection of schistocytes has a critical impact on the timely diagnosis, effective treatment, and positive prognosis of diseases such as thrombocytopenic purpura and hemolytic uremic syndrome. Due to the rapid development of science and technology, laboratory hematology has also advanced. The accuracy and efficiency of tests performed by fully automated hematology analyzers and fully automated morphology analyzers have been considerably improved. In recent years, substantial improvements in computing power and machine learning (ML) algorithm development have dramatically extended the limits of the potential of autonomous machines. The rapid development of machine learning and artificial intelligence (AI) has led to the iteration and upgrade of automated detection of schistocytes. However, along with significantly facilitated operation processes, AI has brought challenges. This review summarizes the progress in laboratory schistocyte detection, the relationship between schistocytes and clinical diseases, and the progress of AI in the detection of schistocytes. In addition, current challenges and possible solutions are discussed, as well as the great potential of AI techniques for schistocyte testing in peripheral blood.

A síndrome HELLP é uma condição na qual a gestante apresenta hemólise, aumento de enzimas hepáticas e plaquetopenia, sendo considerada uma manifestação grave da pré-eclâmpsia. Nos casos em que há um ou dois desses critérios, denomina-se síndrome HELLP parcial ou incompleta. A patogênese ainda carece de completo esclarecimento, contudo acredita-se que esteja relacionada à fatores placentários que desencadeiam uma resposta inflamatória sistêmica exacerbada, além de mutações genéticas que contribuem com o dano endotelial e, consequentemente, podem predispor à condição. Epidemiologicamente, sabe-se que a maior incidência ocorre entre 27 e 37 semanas de gestação, embora possa ocorrer em até 7 dias após o parto. Os principais fatores de risco incluem o histórico de distúrbios hipertensivos em gestações prévias, a nuliparidade e a idade materna superior a 35 anos. Clinicamente, é comum que a paciente manifeste sinais clássicos de pré-eclâmpsia (hipertensão e proteinúria), além de dor epigástrica e no quadrante superior direito, náuseas e vômitos, sendo que a sintomatologia tem início súbito e raramente cursa com remissão espontânea. Vale ressaltar que a síndrome pode ser assintomática e possui diagnósticos diferenciais importantes, como hepatites virais, anemia hemolítica e fígado gorduroso agudo da gravidez. Para o diagnóstico da síndrome HELLP completa, considera-se a elevação das enzimas LDH acima de 600 unidades/L, ALT e AST acima de duas vezes o limite superior, além de contagem de trombócitos inferior à 100.000 x 109/L. O manejo terapêutico envolve o parto imediato, preferencialmente via vaginal, quando a IG ≥ 34 semanas e a conduta expectante, associada à administração de corticosteróides, quando a IG < 34 semanas e houver estabilidade clínica materno-fetal. A terapia medicamentosa inclui o uso de sulfato de magnésio para a prevenção da eclâmpsia e de hipotensores de ação rápida.

A prediction model for the progression from gestational hypertension to pre‐eclampsia complicated with HELLP syndrome

ObjectiveHELLP syndrome is a severe complication of hypertensive disorders of pregnancy that can cause multiple organ dysfunction and maternal death in a short period of time. Although HELLP syndrome is more common in patients with pre‐eclampsia (PE), there is currently no effective way to identify high‐risk individuals who may progress from gestational hypertension (GH) to PE complicated with HELLP syndrome. This study aimed to establish and validate a prediction model for PE complicated with HELLP syndrome, providing a basis for early detection and identification of high‐risk individuals in clinical practice.MethodsThis retrospective case–control study collected data on 326 patients with GH and 139 patients with PE complicated with HELLP syndrome from January 2015 to December 2019. An additional 206 patients with GH and 70 patients with PE complicated with HELLP syndrome who were treated from January 2020 to December 2022 were collected for external validation. General and clinical data were collected, and single‐and multiple‐factor logistic regression analyses were used to screen for independent factors affecting PE complicated with HELLP syndrome. The diagnostic performance of different indicators was evaluated using ROC curves. A prediction model for PE complicated with HELLP syndrome was constructed, and its efficacy was verified using ROC curves.ResultsThe results of single‐factor analysis showed that age, SBP, DBP, MAP, hemoglobin, AST, ALT, cholinesterase, alkaline phosphatase, gamma‐glutamyl transferase, total protein, total bilirubin, direct bilirubin, indirect bilirubin, BUN, UA, creatinine, APTT, international normalized ratio of prothrombin, D‐dimer, fibrinogen, fibrinogen degradation products, Ca, and aspartate‐aminotransferase to platelet ratio index (APRI) were factors influencing PE with HELLP syndrome. The results of multiple‐factor logistic regression analysis showed that MAP, APRI, CHE, FDP, and Ca were independent factors affecting PE complicated with HELLP syndrome. Based on these results, a prediction model was established, with Y = 9.861 + 2.998APRI + 0.055MAP + 0.014FDP − 0.005CHE − 7.452*Ca.ConclusionsThe predictive model for PE complicated with HELLP syndrome includes APRI, MAP, FDP, CHE, and Ca. This model can be used as a quantitative tool for predicting and evaluating the development of GH into PE complicated with HELLP syndrome.
