Minipuberty of human infancy – A window of opportunity to evaluate hypogonadism and differences of sex development?

Abstract: Activation of the hypothalamic-pituitary-gonadal (HPG) axis happens in 3 phases during life. The first phase is during fetal life and is only separated from the second phase, called minipuberty, by the high concentration of placental hormones at birth. The third period of activation of the HPG axis is puberty and is well-described. Minipuberty consists of the neonatal activation of the HPG axis, mainly in the first 1-6 months, where the resulting high levels of gonadotropins and sex steroids induce the maturat… Show more

“…Adult females have little or no breast development and primary amenorrhea [40,41]. Infant boys with CHH often have micro phallus and cryptorchidism (i.e., undescended testes), thus providing the possibility of an early diagnosis [42,43]. CHH can be considered a chronic condition [44] and may lead to many comorbidities, including psychological disorders [45], osteoporosis [46] and increased risk of metabolic defects (e.g., type II diabetes mellitus) [47].…”

“…Minipuberty may represent a “window of opportunity” to evaluate the HPG axis by measuring basal hormone concentrations with no need for stimulation tests in infants with suspected reproductive disorders. Minipuberty provides a unique opportunity to evaluate the spontaneous function of the HPG axis which is lost thereafter for approximately another 10 years until the HPG axis is reactivated in puberty ( 67 ).…”

“…Sexual development, including breast development or testicular enlargement, is initiated through hypothalamic-pituitary-gonadal (HPG) axis reactivation, after initial activation during the fetal and early postnatal periods [12]. However, before pubertal onset, the negative feedback mechanism acts excessively, and gonadotropins, such as luteinizing hormone (LH) and follicle stimulating hormone (FSH), do not increase despite the low serum sex hormone concentration.…”
