Furthermore, micro-PNL, the newest PNL method, enables visual entry in a single session via a special puncture needle, called an "all-seeing needle", by means of a micro-optical imaging system; this method does not require dilation because the nephroscope can be as fine as 4.8 Fr (13,14). This new method not only avoids dilation, which may cause complications such as hemorrhage and perforation, but also shortens operation and fluoroscopy times (14). Micropercutaneous Nephrolithotomy: First 66 Cases ExperienceObjective: To present the initial experience with micro-percutaneous nephrolithotomy (micro-PNL) in patients with kidney stones.Methods: Medical records of 66 patients with kidney stones who underwent micro-PNL between December 2012 and March 2015 were retrospectively reviewed. The demographic data; stone side, location and size; operation and fluoroscopy time; intraoperative and postoperative complications; preoperative and postoperative hemoglobin levels and hematocrit; and operation success of the patient were evaluated. Results:The mean age of the patients was 46.62±13.94 years, and their mean body mass index was 25.77±2.62 kg/m 2 . The mean stone size was 186.7±34.23 mm 2 . The mean operation and fluoroscopy times were 80.46±43.67 min and 10.78±7.14 min, respectively. The mean hemoglobin drop was 0.86 g/dL. Twenty-four intraoperative and 17 postoperative complications were observed. An overall success rate of 95% (including clinically insignificant residual fragments) was achieved. Conclusion:Micro-PNL is a feasible, safe, and effective minimally invasive treatment modality for patients with small kidney stones.

Percutaneous nephrolithotomy is accepted as the standard management approach for kidney stones that are either refractory to extracorporeal shock wave lithotripsy or are >2 cm in diameter. The recently developed micro-percutaneous nephrolithotomy (microperc) technique provides intrarenal access under full vision using an optic instrument with a smaller calibration. A lesser amount of bleeding has been reported with the use of this method. Here we present a case of a bleeding complication on postoperative day 15 after a microperc procedure used to treat a left kidney stone. The complication led to retention of bloody urine in the bladder and required transfusion of 5 units of whole blood.

Sažetak. Cilj: Prikazati naše rezultate u liječenju pacijenata s bubrežnim kamencima metodom miniperkutane nefrolitotripsije (miniPCNL). Pacijenti i metode: Retrospektivnim istraživanjem obuhvatili smo pacijente Klinike za urologiju, Kliničkog bolničkog centra u Rijeci koji su između 1. kolovoza 2015. i 31. prosinca 2016. godine zbog bubrežnih kamenaca liječeni metodom miniPCNL-a. Rezultati: U promatranom razdoblju u našem centru operirano je 6 pacijenata ovom metodom, od kojih je jedan bio s transplantiranim bubregom. U svih pacijenata uspješno je učinjena litotripsija s holmium-laserom. Na kontrolnom RTG-u nije bilo ostatnih fragmenata. U četvoro pacijenata poslijeoperativno je došlo do razvoja febriliteta koji je uspješno liječen antibiotskom terapijom. Niti u jednog pacijenta nije bila potrebna reoperacija, dodatne procedure niti potreba za davanjem krvi. Zaključak: Miniperkutana nefrolitotripsija je minimalno invazivna metoda koja se pokazala uspješnom i sigurnom u liječenju nefrolitijaze.Ključne riječi: endourologija; laser; nefrolitijaza; perkutana litotripsija Abstract. Aim: To present our results in the treatment of nephrolithiasis using mini percutaneous nephrolithotripsy (miniPCNL). Patients and methods: We retrospectively analyzed all patients with nephrolithiasis treated with miniPCNL in Department of Urology, University Hospital Rijeka from August 1 st 2015 to December 31 st 2016. Results: In observed period 6 patients were operated with this novel method and one has transplanted kidney. In allpatients lithotripsy was successfully performed with holmium laser. On the control x-ray the residual fragments were not found in any patients. Postoperatively, in four patients febrility was noticed and successfully treated with antibiotics. Neither the one patient need reoperation, auxiliary procedures or blood transfusion. Conclusion: Mini percutaneous nephrolithotripsy is a minimally-invasive method which is successfull and safe method in the treatment of kidney stones.
