Middle Cerebral Artery Stenosis Is a Major Clinical Determinant in Striatocapsular Small, Deep Infarction
Abstract: The proximal MCA lesion was a common cause of SSDIs in Korean patients. Depending on the existence of an MCA lesion, the clinical course and magnetic resonance imaging feature of the patients with SSDIs were different.

“…Stenotic lesions of the MCA were associated with progressive clinical courses in patients with striatocapsular small, deep infarction. 17 Also, in contrast to our study, Adachi et al reported that small subcortical infarction resulting from large-vessel disease showed more frequent progression than lacunar infarction. 8 However, the progression of neurological deficits was not clearly defined in that study.…”

“…Consistent with recent study results showing that MCA atherosclerosis is an important cause SSI in Asia [13,15], MCA stenosis was shown in 30% of the patients with SSI in our study. There have been debates on the role of MCA diseases on the lesion size or clinical outcome in patients with SSI; although the size of SSI produced by MCA disease was considered larger than that produced by SVD [23], a recent study using DWI showed that the lesion size was not different between the patients with MCA disease and those without [15].…”

“…The wall and lumen of the black blood sequence T 2 WI‐FSE had a high contrast ratio, which was beneficial in defining the boundaries of lumen‐blood and wall‐meninges cerebrospinal fluid. Our study also found that CWS patients were younger, and had a smaller percentage of stroke history than TIA patients, which indirectly supports the view that CWS represents an early lesion of large‐artery atherosclerosis …”

“…At the same time, the 46 CWS patients had a high percentage of large‐artery atherosclerosis risk factors, such as hypertension, smoking, and hyperlipidemia, while those risk factors for small‐vessel lesions, such as diabetes, had a relatively small percentage. This result indirectly indicated that CWS might be related to large‐vessel lesions, which is also suggested by previous research . Large‐artery stenosis was not found by conventional cerebrovascular examination to be associated with early positive remodeling of atherosclerosis.…”
