“Someone to Talk to”: A Qualitative Study of Oncology Trainees' Experience of Mentorship around Moral Distress

Purpose Moral distress is an intrinsic part of healthcare, particularly prevalent in oncology practitioners. Previous studies have suggested mentorship may play a role in combatting moral distress; however, there is a lack of good evidence aimed at understanding trainees’ experience with either mentorship or moral distress, including the intersection between the two. Materials and Methods We conducted a single-centre study in the hermeneutic phenomenological approach at a Canadian academic cancer centre. Six semi-structured interviews with senior oncology trainees were conducted and analysed according to the interpretive profiles hermeneutic phenomenological approach. Results Key findings include the idea that trainees do find mentorship valuable and helpful in navigating moral distress, which is described as common and inevitable, with a number of triggers and factors identified. However, a mentorship relationship must involve mutual respect, understanding, and honesty in order to be valuable. Additionally, engaging in open, honest discussions with mentors, particularly more senior individuals, is seen as a risk-benefit balance by trainees; vertical mentors bring more wisdom and experience, but may also have a greater impact on a trainee’s future. Conclusions This thought-provoking study highlights mentorship as a potential method to combat the troubling phenomenon of moral distress in oncology trainees.

This study aims to explore the experiences of depression survivor during the COVID-19 pandemic to cope with depression. This study used a grounded theory approach, with a single subject who until this research was completed in the recovery phase from depression. The subject was selected for the first reason, she had been diagnosed to experience anxiety and depression and was still in the recovery stage. Second, the subject learned and practiced Kawruh Jiwa Ki Ageng Suryomentaram to help her recover from anxiety and depression. The data collection method used was a semi-structured interview, which was conducted four times, recorded, and transcribed verbatim. Data were analyzed using coding by processing basic data into conceptual data. The results of this study indicated that: 1) the pandemic is a common trigger factor that causes the subject to feel depressed; 2) grief because of the death of both parents is the leading trigger for the subject to experience anxiety and depression. Subject experienced 10 of 17 signs of depression both in aspects: Motor, autonomic hyperactivity, excessive alertness, and reduced apprehension. Research also showed that efforts to ngudari reribet (solving problems of anxiety and depression) are performed in several stages: First, kandha takon both interpersonally (sharing) with others by seeking help from professionals, psychologists, and psychiatrists and kandha takon with an intrapersonal model (nyawang karep), second, nyocoaken raos by examining feelings what she feels herself, the three jawah kawruh (understanding and perceiving the feeling she feels herself ), the four Aku, sak iki, nengkene, ngene, gelem accepting herself today, and mawas diri, introspective efforts to keep her consciousness intact. These findings open up a new space to build the discourse and practice of psychological therapy in anxiety and depression survivors.

Male adolescents of sexual minority groups often present mental health problems and suicidal behaviors. This qualitative study was conducted to explore how a bisexual youth with borderline personality disorder and multiple suicide attempts coped with his intention to die. Analysis was done using the Grounded Theory approach resulted in several themes. First, the pathway to suicidal behaviors; depicting the conflict between parents and the history of mother’s suicide attempts made him felt disconnected with the family. Second, the feeling of not deserving loved although having the need to be loved and to love. Third, the ambivalence of his sexual orientations as he had strong desire to be with a man yet felt happy dating a girl. Fourth, the interpretation of religious values for preventing fatal suicidal act. He perceived that his suicide attempts failed because God wants him to stay alive. The discussion addresses the aspect of religiosity that obstructs suicidal behaviors in bisexual youths
