Melanocytes in Long-standing Sun-Exposed Skin

Abstract: To help distinguish early melanoma from normal sun-damaged skin by quantifying the density, confluence, and depth of follicular penetration of melanocytes in long-standing sun-exposed skin of the face and neck. Design: Case series. Setting: Referral center. Patients: Random selection of 149 patients undergoing Mohs surgery for basal cell and squamous cell carcinomas of the face and neck. Intervention: Frozen-section slides were made from long-standing sun-exposed normal skin and stained with MART-1 (melanoma a… Show more

“…These authors noted Melan-A-positive nests at the dermal-epidermal junction of a patient with discoid lupus, while S-100 and combined tyrosinase/HMB-45 demonstrated melanocytes distributed in a normal pattern. This finding may also account for the dermal cells noted by Hendi [41], as discussed above.…”

“…Hendi and colleagues attempted to distinguish early melanoma from normal sun-damaged skin by quantifying the density, confluence and depth of follicular penetration of melanocytes in the sun-exposed skin of the face and neck [41]. Redundant tissue cones from the repair of 149 Mohs surgical procedures for basal cell carcinoma or squamous cell carcinoma were analyzed by frozen section and stained with Mart-1.…”

“…4,4249 The overall estimate of MD was 955.05 cells/mm 2 (95% CI 880.89–1029.21). The genitalia had the highest MD, followed by the head/neck, upper extremities, lower extremities, and trunk.…”

“…1,2,11,18,20,21 While nesting and pagetoid spread are less commonly found in benign sun-damaged skin, they may occur. 1,2 The findings in our specimens that overlapped with melanoma in situ were focal and isolated and in no instance would the J AM ACAD DERMATOL VOLUME 64, NUMBER 3 pathologist have struggled with the diagnosis. Although our results are reassuring, we concede that criteria remain somewhat subjective and that pathologists should remain cautious in the interpretation of melanocytic margins on sun-damaged skin.…”
