Objective: Electronic (e) prescribing is computerised prescribing. The Government of Kerala has started the e-health initiative. The objective of this study was to obtain the perceptions of prescribing doctors on e-prescription before its implementation. Method: This was a questionnaire based cross sectional study conducted for a period of six months in a Government tertiary care teaching hospital of Kerala. The prescribing doctors of the institution formed the sample population. The response to the structured questionnaire was measured on Likert scale. Data were analysed using SPSS 16. Domain scores of perceived usefulness, ease of use and fitness to the Institution were calculated by averaging the response. Chi square test was done to find the association of score of each domain with independent variables. Results: Two hundred and fifty seven prescribing doctors of age range 22-60 years participated of which 49% were females and 51% males. Response rate was 73% (257/350). Although 70.5% have heard of e-prescriptions only 14% participants had hands on exposure. The mean perceived usefulness, perceived ease of use and fitness were 3.5±0.47, 3.35± 0.49 and 2.95 ± 0.44. Conclusion: Dislike for paper prescriptions was the single most important factor associated with perceived usefulness, perceived ease of use and perceived fitness of e-prescription in the institution. Information technology training and infrastructure development should go hand in hand with implementation of e-prescribing. Key Messages: For successful implementation of electronic prescriptions, training and technologically developed settings is a must. Educating the prescribers on preventable medication errors associated with paper prescriptions can lead to the acceptance of e-prescribing. We present here, a child with an extremely high methemoglobin level caused by inadvertently prepared sodium nitrite and potassium nitrite by a pharmacist. On physical examination, he had tachycardia and tachypnea. He had cyanosis, and mottled and blue-gray skin color. Oxygen saturation was 85% on room air. He was intubated due to seizure and arterial hypotension. Although he had no history of cyanotic heart disease, cyanosis was resistant to 100% oxygen therapy. Venous blood gas sampling revealed metabolic acidosis and methemoglobinemia (86.2%). Methylene blue dye at a dose of 2 mg/kg and ascorbic acid at a dose of 500 mg were administered intravenously and the clinical improvement was observed in 12 hours. Methemoglobinemia can cause severe systemic findings and death. The clinical findings can mimic septic shock. Emergency physicians should have awareness of methemoglobinemia in patients with cyanosis unresponsive to oxygen therapy. İlk fizik muayenesinde hasta taşikardik ve takipneikdi. Hastanın siyanozu ve gri -mavi bir deri rengi vardı. Bilinen bir kardiyak hastalığı olmamasına rağmen, %100 oksijen ile satürasyonu %85 idi. Geldiğinde siyanoz, bilinç değişikliği, konvülziyonu ve hipotansiyonu olan hasta entübe edildi. Alınan venöz kan gazında methemoglobinemi (%86,2) ve metabolik asidoz saptandı. Hastaya metilen mavisi 2 mg/kg intravenöz ve 500 mg intravenöz askorbik asit uygulandı. Takibinde methemoglobin seviyesi 12 saat içinde tedrici olarak geriledi ve klinik düzelme görüldü. Methemoglobinemi, ciddi bulgulara ve ölüme neden olabilecek bir durumdur. Klinik bulgular septik şok ile karışabilmektedir. Acil hekimleri oksijene yanıt vermeyen siyanoz durumlarında methemoglobinemi olabileceğini mutlaka düşünmelidir.

The widespread accessibility of antimicrobials transformed the path of olden times. Many infections beforehand not curable found treatment, for example, tuberculosis. Scientist, common people thought once that humankind has able conquered microbial diseases. Nonetheless, irrational and unnecessary use antimicrobials create resistant microbes throughout the universe. Moreover, the development new antimicrobials have slowed down, particularly for those diseases of developing countries who have very poor financial ability to pay the research overhead of the inventor. Thereafter, currently available antimicrobials antimicrobials are loosing their efficacy and spectrum everyday against resistant microbes. Subsequently, once those diseases were treatable with no antimicrobials or very low profile agents now need the higher form of antimicrobials even multiple agents. As such health care cost is increasing every day which is beyond the economic ability of common people of anywhere of the world. All these 10 developing countries very frequently were observed that overuse and irrational use of antimicrobials. Researchers of these developing countries opined that answer remains in continuing medical education for all health-related stakeholders and appropriate policy to stop irrational prescribing, and promote prudent and rational prescribing and use of medicine, including antimicrobials.
