Medical interventions for fungal keratitis

Abstract

Background Fungal keratitis is a fungal infection of the cornea. It is common in agricultural tropical countries but relatively uncommon in developed countries. Although there are medications available, their effectiveness is unclear. Objectives To examine the effect of different antifungal drugs in the management of fungal keratitis.

“…11 There is no agreed protocol for the treatment of suspected fungal keratitis in the United Kingdom, and a recent review of six randomised controlled trials, all performed in India or Bangladesh, did not demonstrate the superiority of any particular antifungal agent. 25 Amphotericin 0.15% was the principal topical antifungal agent used in our series, followed by 1% econazole. Topical natamycin 5%, a commonly used drug for filamentary fungal infection in many countries, is not generally available for prescription in the United Kingdom.…”

Comparative Antifungal Susceptibility Analysis of Candida albicans Versus Non-albicans Candida Corneal Isolates

“…The authors concluded that based on the available studies, there is no evidence that any particular drug, or combination of drugs, is more effective in the management of fungal keratitis. 14 AMB, a polyene, is a broad-spectrum antifungal agent. 15 It is effective topically for most cases of Candida and other fungi 15 and is frequently the treatment of choice for Candidal keratitis.…”

En la Revisión Cochrane 2008 (39) , se concluyó que tanto la natamicina al 5% como todos los medicamentos evaluados fueron inefectivos en el tratamiento de la queratitis fúngica, evidenciándose la necesidad de realizar nuevos estudios clínicos aleatorizados de terapia. Sin embargo, ninguno de los estudios evaluados en dicha revisión sistemática utilizó anfotericina B ni voriconazol.…”
