Measuring Sleep: Accuracy, Sensitivity, and Specificity of Wrist Actigraphy Compared to Polysomnography

Abstract: This validation quantifies strengths and weaknesses of actigraphy as a tool measuring sleep in clinical and population studies. Overall, the participant-specific accuracy is relatively high, and for most participants, above 80%. We validate this finding across multiple nights and a variety of adults across much of the young to midlife years, in both men and women, in those with and without insomnia, and in 77 participants. We conclude that actigraphy is overall a useful and valid means for estimating total sle… Show more

“…While polysomnography is the gold standard for objectively quantifying sleep disturbances, it is a logistically difficult approach demanding much of the patient and is only a one night "snapshot" of sleep. Actigraphy data is a more cost effective alternative, 43 especially for longitudinal studies and those in the outpatient environment.…”

“…19,20,21 These findings indicate that the contact-free ES sensor may be used as an objective sleep diary. Compared to actigraphy, the ES device has the advantage of being conveniently installed under the mattress, enabling seamless monitoring without the burden of wearing or charging a device.…”

“…Long-term monitoring of sleep requires more practical methods. Use of actigraphy, while a viable alternative, [7][8][9] is limited by the need for costly, specialized equipment and may be considered cumbersome by some patients. A reliable, affordable and user-friendly smartphone-based app that can monitor sleep-wake cycles would be an invaluable addition to the arsenal at the disposal of practitioners of sleep medicine.…”
