Objetivos. Determinar la tendencia y factores asociados al cepillado de dos o más veces por día en niños menores de doce años entre el 2013 y 2018 en Perú. Materiales y métodos. Se realizó un estudio transversal mediante análisis secundario de datos reportados por la Encuesta Demográfica y de Salud Familiar. Se consideró la información de niños con al menos un cepillado al día, considerando como «Adecuada Frecuencia de Cepillado Dental» (AFC) para quienes reportaban de dos a más; se generaron medidas porcentuales por ámbito geográfico, características sanitarias, de vivienda y hogar. Se determinaron los factores asociados a la AFC mediante regresión logística binaria para encuestas complejas, la variabilidad de la tendencia se identificó mediante regresión joinpoint con un 95% de confianza. Resultados. En el 2013, la AFC fue del 79,0% con un aumento a 83,9% para el 2018; se identificó que cuatro de 25 espacios geográficos se mantuvieron sin variación de AFC, y otros cuatro disminuyeron. Todas las características sanitarias mostraron favorecer el cepillado, destacándose que el no compartir el cepillo generó un OR de 2,30, IC 95%: 1,46 a 3,60. La región natural, el tipo de lugar de residencia y el quintil de riqueza afectaron la AFC; la AFC fue mayor según se incrementa el grupo etario. Conclusiones. El porcentaje de AFC ha aumentado en los últimos años, siendo influenciado favorablemente por los aspectos sanitarios; a nivel rural este porcentaje es significativamente menor, algo que también se evidencia en la región sierra.

Factors associated with home birth in Peru: An analysis of the Demographic and Health Survey, 2019

Objective: To determine the proportion and factors associated with home birth in Peru, 2019. Material and methods: Cross-sectional analytical design study where the 2019 Peruvian Demographic and Family Health Survey was analyzed. The association was evaluated using Poisson Regression, supplemented with the crude prevalence ratio (cPR) and adjusted (aPR). Results: The records of 18,401 women were evaluated, where 5.39% (95%CI:4.83-6.03%) presented home birth. The probability of a home birth occurs increases when the pregnant woman is from the andean (aPR:1.24; 95%CI:1.02-1.48) and amazon region (aPR:1.38; 95%CI:1.16-1.64), resides in rural areas (aPR:3.34; 95%CI:2.61-4.29), presents less than 6 prenatal care (aPR:1.66; 95%CI:1.39 -1.96), it is very poor (aPR:9.62; 95%CI:5.13-18.1) or poor (aPR:2.39; 95%CI:1.26-4.52), it has not studied (aPR:2.66; 95%CI:2.02-3.50) or reached primary education (aPR:2.18: 95%CI:1.85-2.58) and has 2 children (aPR:1.64; 95%CI:1.46-1.85) or 3 or more children (aPR:2.18; 95%CI:1.67-2.87). On the other hand, having higher educational instruction (aPR:0.49; 95%CI:0.31-0.78) is associated with a lower probability of a home birth. Conclusions: There is a low proportion of home births; however, this indicator increases significantly according to various geographical, sociodemographic, and obstetric factors that have been identified.

Background Like many other Latin America- and Caribbean countries, Peru has introduced a tax-financed health insurance scheme called “Sistema Integral de Salud (SIS)” to foster progress towards Universal Health Coverage. The scheme explicitly targets the poorest sections of the population. Our study explores levels of health insurance coverage and their determinants among Peruvian women following the introduction of SIS. We wish to determine the extent to which the introduction of SIS has effectively closed gaps in insurance coverage and for whom. Methods Relying on the 2017 round of ENDES (Encuesta Nacional Demográfica y de Salud Familiar) survey, we analyzed data for 33,168 women aged 15–49. We used multinomial logistic regression to explore the association between health insurance coverage (defined as No Insurance, SIS, Standard Insurance) and women’s socio-demographic and economic characteristics. Results Out of the 33,168 women, 25.3% did not have any insurance coverage, 45.5% were covered by SIS and 29.2% were covered by a Standard Insurance scheme. Women in the SIS group were found to have lower educational levels, live in rural areas and more likely to be poorer. Women in the Standard insurance group were found to be more educated, more likely to be “Spanish”, and to be wealthier. Most uninsured women appeared to belong to a middle class, not poor enough to be eligible for SIS, but also not eligible for standard insurance. Conclusions Our study confirms that SIS has been effective in increasing coverage among vulnerable women, with coverage rates comparable with those observed among men. Nevertheless, on its own, it has proven to be insufficient to ensure universal coverage among women. Further reforms are needed to ensure that coverage is extended to all population groups.
