Background:In the past few years, growing interest was given to the relationship between the dental occlusion and the body balance. While most research focused on this relationship at static conditions, it is evident that the contribution of the sensory information for balance control is different depending on the environmental constraints. Research question:The aim of the present paper was to elucidate whether the stomatognathic system (SS) contributes differently on body balance regulation according to the presence of external disturbances. Methods:Literature regarding the different sources involved in the proprioceptive information to the SS was reviewed. The influence of dental occlusion on balance control at different external environments was then explored. Results:The main findings are: (a) a plausible evidence between the masticatory and cervical muscles can be described; (b) a reciprocal connection between the trigeminal and vestibular nuclei supports the influence of the SS on body balance; (c) traditionally, research involving the relationship between the SS and balance control has focused on strictly controlled situations, thus, ignoring the sensory reweighting which occurs depending on the external disturbances; and (d) the afferences of dental occlusion for balance control seem strengthened when more difficult conditions are present. Conclusion:Results of the present review suggest that afferent signals from dental occlusion effectively contribute to balance control when more external perturbations are present, that is unstable support surface, fatigue and tasks being performed.However, more studies are needed to elucidate the mechanisms by which dental occlusion may influence balance control focusing on different external environments. Military pilots are characterized by peculiar job conditions related to intense accelerative stresses. For this, they frequently report work-related neck and back pain and are affected by bruxism. The aim of this case-control study is to analyze the occlusion time (OT) in a sample of military pilots affected by bruxism, compared with healthy non-pilot subjects. 14 military pilots, all males (mean age 35.14 ± 2.32 y), affected by bruxism, were compared with an age matched control group of 14 male subjects (mean age 32.29 ± 4.84 y). The T-Scan computerized occlusal analysis system (Tek-Scan Inc., Boston, MA, USA) was used to record the subjects’ occlusion times during 4 mandibular opening-closing movements. Military pilots affected by bruxism showed a statistically significant lower (reduced of 85%) mean OT, compared with control subjects (p < 0.0001). Military pilots are characterized by a highly reduced occlusion time compared to healthy non-pilot subjects. This feature could not be only related with the presence of bruxism but with their particular clinical job-related condition that causes different dental and stomatognathic system problems. Further studies are encouraged to enhance the scientific knowledge in this field.

Não há um consenso na literatura quanto à associação entre postura craniocervical (PCC), disfunção temporomandibular (DTM) e oclusão dentária. Ainda assim, admite-se que a articulação temporomandibular está intimamente envolvida com a biomecânica cervical e cintura escapular. Objetivo: Traçar o perfil das associações entre PCC, DTM e oclusão dentária. Método: Estudo observacional em corte transversal realizado em universitários, de ambos os sexos, com idade entre 18 e 40 anos. Os voluntários foram avaliados pelo RDC/TMD, biofotogrametria, sendo esta realizada através do Software de Avaliação Postural (SAPO) pelos ângulos A1(tragus-horizontal), A2 (tragus-C7-horizontal), A3 (tragus- acrômio-vertical) e distância toraco cervical, além da análise quanto ao tipo de oclusão dentária. O tratamento dos dados foi feito a partir de medidas de tendência central, razão de prevalência e análise de correspondência múltipla assimétrica. Resultados: Foram avaliados 88 voluntários com mediana da idade de 22 anos, sendo 27 do sexo masculino e 62 do feminino. A amostra foi dividida em quatro grupos, de acordo com a presença de DTM, padrão de dor à palpação, cefaléia, PCC e oclusão. As maiores associações observadas foram: DTM tipo II com a distoclusão tipo dois, DTM tipo I com flexão de cabeça e retificação da coluna cervical; mesioclusão com alinhamento da coluna cervical e indivíduos sem dor com anteriorização da cabeça e distoclusão tipo um. Conclusão: Este estudo demonstrou que a DTM não interfere na postura da cabeça, mas que a má oclusão pode alterar a postura craniocervical nos planos sagital e frontal além de aumentar a lordose cervical.

Abstract: The findings of this study seem to suggest that the composite parameter of the SOT is not sensitive in analyzing the influence of the stomatognathic system on the postural balance of civilian and military pilots.
