Lymphadenectomy for the management of endometrial cancer

Abstract

This review found no evidence that lymphadenectomy decreases risk of death or disease recurrence compared with no lymphadenectomy in women with presumed stage I disease. Evidence on serious adverse events suggests that women who undergo lymphadenectomy are more likely to experience surgery-related systemic morbidity or lymphoedema/lymphocyst formation. Currently, no RCT evidence shows the impact of lymphadenectomy in women with higher-stage disease and in those at high risk of disease recurrence.

Other EC subtypes (such as adenosquamous, clear cell and serous carcinomas) usually tend to present at a more advanced FIGO staging and tend to have a poorer patient prognosis, as they are typically high-grade tumors (G3 grading) [6]. The results of our study revealed similar findings, as the histopathological diagnosis of endometrioid carcinoma was a good prognostic factor for patient PFS and OS.…”

Other authors, however, suggested that a selective LND strategy based on a preoperative prediction model is shown to be more costeffective than routine LND for EC [17]. The differences are probably due to the inclusion of different risk tumors and the differences in National Health Service budgets.…”
