Comparative Study of Fentanyl vs Dexmedetomidine as Adjuvants to Intrathecal Bupivacaine in Cesarean Section: A Randomized, Double-Blind Clinical Trial

Abstract: Purpose: Effective postoperative analgesia is essential in cesarean section. This study aimed to compare postoperative analgesia and hemodynamic changes after intrathecal use of fentanyl or dexmedetomidine combined with bupivacaine. Patients and Methods: This study involved 110 pregnant women with ASA I and II and gestational age ≥37 weeks who were candidates for elective cesarean section. They were randomly divided into two groups of 55; Group B-D received 10 mg bupivacaine (0.5%) + 5 μg dexmedetomidine and G… Show more

“…Additionally, patients receiving dexmedetomidine had a faster onset of sensory action (1.54 ± 0.38 minutes) which was statistically significant. Our findings are consistent with those of Khosravi et al [ 8 ] and of Shukla et al [ 9 ] studies that also found a better onset time for 5 µg dexmedetomidine.…”

“…Many studies showed that adding Dex to local anesthetics enhanced the effectiveness of central and peripheral nerve blocks. Dex has been reported to significantly extend the analgesia time when used as an adjuvant in epidural block, 20 PVB, 13,21 subarachnoid block, 22 and brachial plexus block analgesia. 23 A systematic review of 12 RCTs found that epidural Dex was associated with prolonged duration of analgesia, short onset of sensory block, decreased need for rescue analgesia, and higher sedation scores compared to control treatment.…”

“…The reason for a prolonged duration of analgesic effect after TAP blockade may relate to the fact that the TAP is relatively poorly vascularized, and therefore drug clearance may be delayed. Biswas et al [ 26 ] and Khosravi et al [ 27 ] compared the analgesic efficacy of intrathecal fentanyl with hyperbaric bupivacaine after caesarean delivery, showed no significant differences in neonatal Apgar scores at 1 min and 5 min. In our study, also there were no differences in neonatal Apgar scores at 1 min and 5 min.…”
