[Purpose] The aim of this study was to test the hypothesis that Lee Silverman Voice Treatment-BIG decreases the negative impact of hypokinesia on dual task performance in persons with Parkinson’s disease. [Subjects and Methods] The records of 114 patients with Parkinson’s admitted to outpatient rehabilitation at a suburban hospital were reviewed. Demographics and data for 8 outcome measures were extracted for subjects that completed 14 of 16 sessions of BIG. 93 of these subjects had records of pre and post-test Timed Up and Go, Timed Up and Go Motor, and Timed Up and Go Cognitive scores. Average age was 68.4 years (SD=10.6) and average disease duration was 4.9 years (SD=5.3). [Results] Subjects demonstrated statistically significant improvements for Timed Up and Go (3.3 SD=4.5), Timed Up and Go Motor (4.4 SD=5.8) and Timed Up and Go Cognitive (4.7 SD=5.4). Concurrent motor and cognitive performance remained stable. Dual task cost decreased at a statistically significant level for Timed Up and Go Cognitive (7% SD=31%) but not Motor (4% SD=32%). [Conclusion] These findings suggest that cueing strategies associated with LSVT BIG become internalized and decrease the negative impact of hypokinesia on mobility and cognitive performance while performing two tasks simultaneously in persons with Parkinson’s.

BACKGROUND: Parkinson ś disease (PD) is characterized by small, slow movements affecting balance and gait. Lee-Silverman-Voice-Treatment-(LSVT)-BIG is an intensive intervention aiming to restore normal amplitudes of movements. This study aimed to examine whether LSVT-BIG influences dynamic balance and gait in persons with PD. METHODS: We used a Single-Subject-Experimental-Design with three phases; phase A (2 weeks) with no treatment, phase B (4 weeks) with LSVT-BIG-treatment and LSVT-BIG-hometraining, and phase C (7-8 weeks) with LSVT-BIG-home-training. Two persons with PD participated. Outcome measures included the Mini-Balance-Evaluation-Systems-Test (Mini-BESTest) with its four subcomponents ("anticipatory postural adjustments", "postural responses", "sensory orientation", "balance during gait"), the Step-Test (ST) and 10-Meter-Walk-Test-fast (10MWT-fast). The participants were assessed 6, 4 and 8 times during phase A, B and C, respectively. During phase B, they received 16 individual 1-hour LSVT-BIG-treatment sessions and completed between 28 and 39 1-hour sessions LSVT-BIG-home-training. During phase C, they were encouraged to complete one session of home-training every day. RESULTS: The Mini-BESTest-total-score, "balance during gait", ST and 10MWT-fast improved significantly in both participants. One participant demonstrated improved "postural responses". Two of the Mini-BESTest subcomponents were not sensitive to possible improvements. CONCLUSION: LSVT-BIG may have a positive effect on dynamic balance and gait-speed in persons with PD.

ZusammenfassungDie Bewegungsamplitude ist einziger Trainingsfokus bei LSVT-Konzepten und Antwort auf die hyperkinetische Symptomatik bei Parkinson. Innerhalb der Sprechtherapie eingeführt (LSVT-LOUD; Lee Silverman Voice Treatment), wurde dies als motorisches Trainingskonzept im letzten Jahrzehnt zusätzlich auf die Bewegungstherapie bei Parkinson als LSVT-BIG ausgeweitet. Der vorliegende Artikel beschreibt das Trainingskonzept LSVT-BIG vor dem Hintergrund anderer Evidenz- basierter, physiotherapeutischer Trainingsverfahren für Menschen mit Parkinson, mit dem Ziel motorische Fähigkeiten und die motorische Selbstständigkeit zu verbessern.Die sensorische Kalibrierung, der zweite Aspekt der LSVT-Konzepte, ermöglicht es den Patienten wieder normale Lautstärke und Bewegungsausschläge als nicht zu laut oder zu groß zu empfinden. Die Kalibrierung setzt sich dadurch mit den Widerständen auseinander, die einem generalisierten Einsatz physiologischer Amplituden im Alltag entgegenstehen. Sowohl das Training als auch die didaktische Vermittlung des Konzeptes sind auf Parkinson-Patienten zugeschnitten. Die Implementierung in den klinischen Alltag bleibt eine Herausforderung, die Akzeptanz für diesen anspruchsvollen Therapieansatz wächst jedoch vielversprechend.
