OBJECTIVEWe aim to analyze trend of syphilis and factors associated with recurrent episodes of syphilis among adults and adolescents attended in a STI/AIDS reference centers in Campinas, state of São Paulo, 2004 to 2012.METHODSMedical records, pharmacy data, and notification database were accessed to analyze trends of syphilis and sociodemographic, epidemiological and clinical variables associated with reinfection of syphilis. After univariate analysis, a hierarchical logistic regression model was adjusted to analyze variables associated with more than one episode of syphilis (dependent variable). First step (sex, age, and years of schooling) were tested and in the second, epidemiological and clinical variables.RESULTSA total of 1,009 episodes of syphilis were identified among 860 adolescents and adults, 117 individuals (13.6%) presented with more than one episode of syphilis. Factors associated with more than one episode of syphilis were sex (male) (OR = 4.28; 95%CI 1.31–14.0), age (OR = 1.02; 95%CI 1.00–1.04), homosexual/bisexual orientation (OR = 2.29; 95%CI 1.22–4.32), HIV coinfection (OR = 3.54; 95%CI 2.22–5.63), and absence of STI symptoms at the time of syphilis diagnostic (OR = 1.70; 95%CI 1.03–2.80).CONCLUSIONSThe number of cases of syphilis and proportion in relation to STI increased in recent years in a specific population attended in a STI/AIDS reference centers in Campinas. Association with HIV, homosexual/bisexual orientation and the silent clinical characteristic of cases confirm the necessity to implement more aggressive strategies to prevent the occurrence of syphilis and other STI in specific populations with higher disease risk.

Recebido em: 05 de novembro de 2018 -Aceito em: 30 de maio de 2019 RESUMO: Este trabalho tem como objetivo caracterizar os casos de tuberculose e neurotoxoplasmose nos pacientes HIV/Aids atendidos no município de Porto Velho -RO. As análises foram realizadas entre os anos de 2014 a 2018 no SAE do município e no hospital CEMETRON, no total foram realizadas 201 análises. Desses, 17% (34/201) tinham diagnóstico positivo para tuberculose e 11% (21/201) tinham diagnóstico para neurotoxoplasmose. A correlação clínica mostrou que 38% dos pacientes com tuberculose tinham célula TCD4 + entre 50 e 200 células/mm 3 , dos que tiveram neurotoxoplasmose, 71,5% tinham células TCD4 + < 50 células/mm 3 . Entre os anos de 2014 a 2018 muitos pacientes foram à óbito, 35% tinham diagnóstico para tuberculose e 24 % tinham diagnóstico para neurotoxoplasmose. Cerca de 40% dos pacientes não faziam uso da terapia antirretroviral e 44% dos pacientes faziam uso irregular. As coinfecções apresentadas nesse estudo, podem ser evitadas com a regularidade do tratamento antirretroviral que é gratuito no Brasil, necessitando de maior atenção da saúde pública e o desenvolvimento de um programa de maior conscientização ao tratamento para os portadores do vírus HIV. ObjectiveTo determine the prevalence and epidemiological characteristics of inmates diagnosed with infectious diseases living in a region with a high number of prisons, São Paulo, Brazil.DesignThis is a retrospective and descriptive study conducted from November 2017 to October 2018.SettingPrisons located in the western and northwestern regions of São Paulo, Brazil.MethodsWe conducted a retrospective analysis on infectious diseases and coinfections (HIV, hepatitis B virus (HBV), hepatitis C virus (HCV), syphilis and tuberculosis (TB)) of inmates from 28 prisons. Inmates were previously diagnosed following the protocol for control and surveillance of infectious diseases, through laboratory or imaging methods. A questionnaire was completed by the healthcare staff. Prevalence was obtained by dividing the number of individuals with positive results by the number of inmates in each prison. Locations of prisons were obtained and maps were constructed using geographic information systems.ResultsA total of 741 of 37 497 inmates (1.97%) were diagnosed with HIV, HBV, HCV, syphilis or TB. HIV was the most prevalent infectious disease (0.68%), followed by TB (0.66%), syphilis (0.2%), HCV (0.2%) and HBV (0.04%). For all of these diseases, prevalence rates varied from very low to high (3.11% and 2.45%) for TB and HIV, respectively, in the five prisons where they were most prevalent. HIV-syphilis was the most associated coinfection (OR, 63.7; 95% CI 41.4 to 96.7). Three of those diagnosed with the infection were female and the ratio of female to men was 0.004:1.ConclusionsOur findings demonstrate that the number of cases of infectious disease among inmates in the northwestern and western region of São Paulo is probably underestimated, with lower rates of HCV, HBV and syphilis. This represents a challenge to prisoners’ health. Improvements in diagnosis, mainly to reduce viral hepatitis, are crucial with benefits for inmates and the general population.
