Long-term behaviour of the Charnley offset-bore acetabular cup

Abstract: We report the long-term radiological results of 58 total hip arthroplasties (THA) using the Charnley offset-bore acetabular socket. Wear was measured at four sites and radiolucent lines and possible migration were recorded. Four cups were retrieved at revision and were examined using light microscopy, SEM and X-ray microanalysis. At a mean follow-up of seven years the mean wear in the DeLee and Charnley zone I was 0.4 mm and in zone II 0.26 mm. The wear rate was 0.06 and 0.04 mm/year, respectively. Progression … Show more

“…25,[35][36][37][38][39][40][41] Others have advocated proximal or medial placement of the acetabular component, sometimes even fracturing the medial wall. 38,[42][43][44][45] However, these methods compromise the biomechanics of the hip. 1,46 The centre of rotation may be considerably displaced and/ or the use of smaller components may give rise to a higher rate of wear.…”

“…The patients were followed postoperatively for 20-32 years. • Long-term behaviour of the Charnley offsetbore acetabular cup (1998) J Bone Joint Surg Br [ 46 ] The Charnley offset-bore socket is an extra small implant with a face diameter of 35 mm and an eccentric cavity for the femoral head. The overall survival rate was 93.1 % (±6.5 %) at 10 years and 56.1 % (±13.3 %) at 23 when 22 hips remained at risk.…”

“…In order to address this problem the acetabular component with an offset bore has been used, with survival rates of 81.6% at ten years and 46.9% at 20 years. 37 The use of uncemented press-fit acetabular components has been reported to give favourable results. 14,38 Quick and adequate biological fixation by bone ingrowth is a prerequisite for successful longevity of uncemented acetabular components.…”
