Glucocorticoids have been used extensively in the management of patients with renal disease. The beneficial effect of glucocorticoids on renal disease is probably mediated via the suppression of immune function, although some evidence exists for a direct influence of corticosteroids on glomerular function. A myriad of glucocor-ticoid complications have been reported, including: skin, eye, bone, blood and adipose tissue changes, as well as growth retardation, hypertension and an increased susceptibility to infection. A review of the efficacy of glucocorticoid use demonstrates benefit in a small number of glomerulonephritides (minimal change nephrotic syndrome, systemic lupus erythematosus, polyarteritis nodosa and renal transplantation). Since being introduced in the early 1950s, glucocorticoids have proven to be of limited usefulness in the treatment of glomerulonephritis.

“…The introduction of adrenocorticotropic hormone and cortisone in the 1950s contributed to an even greater decrease in mortality (to 9%), which was noted to occur in association with dramatic resolution of proteinuria. 2 Diagnosis of nephrotic syndrome requires the presence of edema, massive proteinuria (>40 mg/m²/hour) or a urine protein/creatinine ratio (>2.0 mg/mg) and hypoalbuminemia (<2.5 gm/dl). 3,4 The annual incidence is 2-3 cases per 100000 children per year and higher in underdeveloped countries resulting predominantly from malaria.…”
