Local Anesthetic Systemic Toxicity

Abstract: Local anesthetics are commonly used in the perioperative environment to facilitate surgical procedures or to provide postoperative pain management for patients. The use of local anesthetics, however, introduces the risk of complications resulting from local anesthetic systemic toxicity and the risks of increased morbidity and mortality for the surgical patient. Systemic toxicity from the injection or overdose of local anesthetics is a rare but potentially fatal complication that occurs in less than 1 in 1,000 … Show more

“…Early signs and symptoms of LAST include a metallic taste, tinnitus, perioral paresthesia, lightheadedness, dizziness, and confusion . Symptoms can appear within 30 seconds to several hours after the anesthesia professional injects the local anesthetic; typically, symptoms appear within minutes . For short procedures (eg, less than 30 minutes), personnel may not identify LAST until the patient is in the PACU; the onset of symptoms will vary from patient to patient.…”

“…U hemijskoj strukturi lokalnih anestetika postoje hemijske komponente: aromatični prsten, središnji lanac i terminalni amini (Slika 1). Aromatični prsten određuje rastvorljivost lokalnih anestetika u lipidima 3 . Na osnovu hemijske strukture lokalni anestetici mogu biti amidi (lidokain, mepivakain, bupivakain, levobupivakain, ropivakain, artikain) i estri (kokain, prokain, tetrakain, benzokain).…”

“…Sistemska toksičnost lokalnih anestetika (LAST) predstavlja životno ugrožavajuću komplikaciju 5 . Lokalni anestetici ometaju funkciju svih organa u koje se prenose impulsi, blokiranjem sprovođenja impulsa kroz aksone u perifernom nervnom sistemu 3 . Povećanjem koncentracije lokalnih anestetika ispoljava se njihov efekat i na centralni nervni sistem, blokadom kortikalnih inhibitornih puteva.…”
