Liver Resection for Hepatocellular Carcinoma on Cirrhosis

Abstract: Child A patients with single nodules are the best candidates for liver resection. Tumor recurrence is strictly linked to the status of the underlying liver disease, and a preoperative AST level equal to 2N seems to be a sensitive cutoff among patients with different risks of recurrence. HCV-positive patients with AST levels above 2N have the highest risk for intrahepatic recurrence and should be monitored carefully or offered alternative treatments.

“…Tumor recurrence complicates 70% of cases at 5 years, reflecting either intrahepatic metastases (true recurrences) or the development of de novo tumors [31,[52][53][54][55][56][57][58]. These entities can be differentiated by means of comparative genomic hybridization, integration pattern of hepatitis B virus, DNA fingerprinting using loss of heterozygosity assays, or DNA microarray studies [64][65][66].…”

“…The main predictors of survival and recurrence after resection are summarized in Table 2 [11,31,[52][53][54][55][56][57][58]. The refinement of assessment of liver function has moved from the gross determination of Child-Pugh class to a more sophisticated measurement of indocyanine green retention rate at 15 min (ICG15) in Japan [59], or hepatic venous pressure gradient (HVPG) P10 mmHg as a direct measurement of relevant portal hypertension in the West [60].…”

“…Long-term survival remains poor due to the high incidence of intrahepatic ROD, which occurs (at 5 years) in 60 -100% of patients treated with surgical resection (5). By contrast, the prevalence of ROD after OLT is much lower, and the 5-year survival rate is Ͼ70% (6,7).…”

“…There have been correlative studies exploring the prognostic factors of HCC recurrence after curative resection [15,16]. Among those clinicopathological factors, characteristics representing pathological invasiveness are the most consistent indicator of long-term prognosis.…”
