Liver Fibrosis: Noninvasive Assessment with MR Elastography versus Aspartate Aminotransferase–to-Platelet Ratio Index

Abstract: Large A(z) values for elasticity (>0.990 for scores > or = F2, > or = F3, and F4) show that MR elastography was accurate in liver fibrosis staging and superior to biochemical testing with APRIs.

“…The theoretical advantages of these methods include the ability to analyze almost the entire liver and the applicability to patients with obesity or ascites. Preliminary studies in human subjects have confirmed the feasibility of these techniques for quantitative assessment of hepatic fibrosis [115][116][117][118][119]. In addition, a recent prospective comparative study in 96 patients with chronic liver disease [120] has suggested that MR elastography had a better diagnostic accuracy than TE for the diagnosis of significant fibrosis (AUROC: 0.99 vs. 0.84, respectively P \ 0.05).…”

“…8 A subsequent prospective comparison of MRE with APRI showed superiority for MRE in liver fibrosis staging. 135 Another study showed that MRE could discriminate between advanced fibrosis (METAVIR F2-F4) and mild fibrosis with a sensitivity of 86% and specificity of 85%. 136 …”

“…Other elastograms acquired in this study appear similar with regard to anatomical features and resolution. In contrast to the classic display of elastograms in MRE [12,13,32], the maps of mechanical parameters in • " Fig. 3 are not masked for excluding noise or regions of unreliably reconstructed parameters.…”
