“…It seems that patients having GKRS-treated foci surrounded by brain parenchyma were more likely to suffer from posttreatment complications. Further, our target volume was smaller than that in previously published studies [1,29,30,31,32]. Contrast-enhancing areas are dense agglomerations of tumor cells with pathological blood supply.…”

“…Since the overall survival observed in our group - 25.6 months - was exceptionally long compared to that of other published studies, it might lead to the conclusion that the presented group was extremely selected and not representative of GBM patients [1,10,26,30,32,33,34]. Pyrosequencing became available in our center in autumn 2013; thus, MGMT promoter methylation results were not known at the time of treatment decision.…”

“…Sirin et al (14) had an improved median OS of 21 months as compared with the previously mentioned two investigators (3,5), while using the regimen of FEBRT and TMX in 58% followed by LINAC SRS. The GBM patients of Sirin et al underwent more STRs and fewer GTRs than those of Elliott et al (5), however, Sirin et al used a 3-mm margin around the contrast-enhancing margin on T1-weighted magnetic resonance imaging (MRI) at the time of SRS treatment.…”

“…The GBM patients of Sirin et al underwent more STRs and fewer GTRs than those of Elliott et al (5), however, Sirin et al used a 3-mm margin around the contrast-enhancing margin on T1-weighted magnetic resonance imaging (MRI) at the time of SRS treatment. This was "to cover potential microscopic expansion of the recurrent tumor" (14). Also, in support of the use of this extended margin, Park et al stated that "tumors tend to progress within 2 cm of the contrast-enhancing edge"(11).…”

“…However, disease recurrence is exceedingly common despite multimodal management. Repeat surgery, systemic agents and RT may be used in the recurrent setting as salvage therapeutic options, nevertheless, the clinical course is typically progressive with almost all patients ultimately succumbing to their disease [3][4][5] .…”
