Laparoscopic Marsupialization of Massive Renal Cyst

Abstract: Laparoscopic approaches to urological problems continue to increase. We describe laparoscopic marsupialization and fulguration of a symptomatic giant renal cyst. Transient obstruction at the marsupialized window illustrated the importance of using omental or epiploic fat to maintain patency. Complete resolution of the cyst and hydronephrosis ensued.

“…The portion of the cyst lying directly on the kidney surface was sealed using a monopolar hook to prevent fluid from being produced by the cyst lining. 3 The cyst edges were sealed and the perirenal fat was placed on the bottom of the cyst (wadding technique). 4,5 Blood loss was minimal.…”

“…59 Open surgical cyst unroofing is associated with a morbidity of 8 to 16%, and the hospital stay averages up to 9 days for this procedure. 32,60 This has largely been supplanted by laparoscopic unroofing, which is much less invasive, [60][61][62][63] but also requires hospitalization up to 3 days. 64 Laparoscopic unroofing is done through either a transperitoneal or retroperitoneal approach depending on cyst location.…”

“…Laparoscopic adrenalectomy was initially applied in 1992 by Gagner et al ( 7 , 8 ). Thanks to developments in imaging modalities, and popularization of minimally invasive interventions, laparoscopic intervention for renal cysts has taken its place among treatment modalities ( 9 ). The greatest disadvantages of laparoscopic surgery which has been introduced with a dazzling speed into urology practice for the last 20 years consist of its higher cost, and longer learning curve.…”
