RESUMO: Objetivou identificar as complicações em pacientes renais crônicos submetidos à hemodiálise e correlacioná-las aos fatores sociodemográficos e clínicos. Estudo transversal, com 200 pacientes em uma clínica de nefrologia no Nordeste do Brasil. Utilizou-se um formulário para a coleta de dados, que ocorreu durante março e abril de 2015. Na análise inferencial, foram aplicados testes de Qui-Quadrado e Exato de Fisher, e U de Mann-Whitney, na associação entre variáveis nominais e numéricas. As associações estatísticas foram: hipotensão com idade, sexo e ganho de peso interdialítico; calafrios com sexo e ganho de peso interdialítico; vômito com sexo, sítio de diálise e KT/V; cefaleia com sexo e idade; tontura com anos de estudo, religião e ganho de peso interdialítico; arritmia e idade; diarreia e tempo de hemodiálise; dor abdominal e ganho de peso interdialítico; sudorese e idade. Conclui-se que complicações durante a hemodiálise podem sofrer influência de fatores sociodemográficos e clínicos. COMPLICAÇÕES EM PACIENTES RENAIS CRÔNICOS SUBMETIDOS COMPLICATIONS IN PATIENTS WITH CHRONIC RENAL FAILURE UNDERGOING HEMODIALYSIS*ABSTRACT: The present study aimed to identify complications in patients with chronic renal failure undergoing hemodialysis and correlate them to socio-demographic and clinical factors. Cross-sectional study with 200 patients conducted in a nephrology clinic in the Northeast of Brazil. Data was collected in March and April 2015 through a structure questionnaire completed by the participants. Chi-square and Fisher's Exact tests were performed in inferential analysis, and Mann-Whitney U test was used to assess the relationship between categorical (nominal) and numerical variables. The statistically significant associations were hypotension with age, gender and interdialytic weight gain; chills with gender and interdialytic weight gain; vomiting with gender, dialysis site and Kt/V; headache with gender and age; dizziness with years of schooling, religious belief and interdialytic weight gain; arrhythmia and age; diarrhea and length of time on dialysis treatment; abdominal pain and interdialytic weight gain; sweating and age. It is concluded that complications during hemodialysis can be affected by socio-demographic and clinical factors.

ÖzAMAÇ: Son dönem böbrek yetmezliği (SDBY) tanısı ile izlenen olguların acil hemodiyalize (HD) alınması morbidite ve mortaliteyi ciddi oranda artırmaktadır. Çalışmamızda, rutin HD programında olan hastaların acil HD endikasyonlarını, kalıcı kateter ve arteriyovenöz fistülü (AVF) olan hastaların özelliklerini sorgulamayı amaçladık. GEREÇ ve YÖNTEMLER:Kasım 2013 -Kasım 2015 tarihleri arasında acil servise başvurusunun ardından acil HD'e alınan 18 yaş üstü 242 hastanın verileri, geriye dönük olarak incelenmiştir. Hastaların sosyodemografik özellikleri, komorbid hastalıkları kaydedilmiştir. Dış merkezden HD amacı ile sevk edilen hastalar, fazla ilaç ve toksik madde alımı olan hastalar ve yoğun bakım ünitelerinde HD'e alınan hastalar çalışmaya dahil edilmemiştir.BUlGUlAR: Çalışmaya (% 50,4) 122 erkek, (%49,6) 120 kadın hasta alındı. Yaş ortalaması 61,7 yıldı. Çalışmadaki hastaların %47,1'inde diyabetes mellitus (DM), %63,1'inde hipertansiyon (HT) saptandı. Hastaların ortalama HD'e girme süresi 37,6 ay olarak bulundu. Hastaların %55'inde AVF mevcut iken, %45'i tünelli kateter ile HD'e girmekteydi. Acil servise başvuru şikayetleri sıklık sırasıyla nefes darlığı (%38,2), bulantı-kusma (%20,2), serebrovasküler olay (SVO) (%7,02), kateter ilişkili sorunlar (%6,2) ve diğer nedenler olarak saptandı. Hastaların acil HD endikasyonları ise sıklık sırasına göre; hipervolemi (%31,8), hiperkalemi (%22,7), üremik bulgular (%21) ve metabolik asidoz (%19,2) olarak saptandı. Her iki grup arasında yaş ile birlikte kalıcı tünelli kateter kullanımı artmış olarak saptanmakla birlikte (p<0,01), ortalama HD'e girme süresi AV fistülü olan hastalarda daha uzun ve istatistiki olarak anlamlı saptandı (p<0,01). SONUÇ: SDBY nedeni ile HD'e giren hastalarda acil servise başvuru oranı hastanemiz çevresinde ciddi oranda yüksektir. Acil HD'e alınan hastalarda kateter kullanımı tüm HD hastalarına göre ciddi oranda yüksektir. Rutin HD etkinliğinin değerlendirilmesi, damar erişim problemlerinin sorgulanması ve düzenli nefrolog takibi çok önemlidir. Aim. Intradialytic exercise (IDE) improves hyperphosphatemia management in hemodialysis (HD) patient in addition to other clinical outcomes. The aim of the study is to present the strategies needed to integrate such a protocol in an HD unit in UAE and patients' baseline characteristics. Methods. The largest HD unit in Sharjah emirate was chosen. All eligible patients (n = 57) in the unit were included. Patients were stable adults HD patients who served as their own controls. The intervention included an aerobic low intensity IDE of 45 minutes per HD session, tailored to each patient's fitness scale (BORG scale) for 6 months. Patients were educated on the importance of exercise. Outcome measures were barriers to exercise, serum phosphorus (P), urea reduction ratio (URR), malnutrition inflammation score, quality of life (QOL using euroqol5) collected at baseline and post intervention. Results. A total of 41 patients completed the study, 61% were males and 90.2%, 53.7% and 14.6% suffered from hypertension, diabetes and cardiovascular disease, respectively. Hyperphosphatemia was prevalent among 75% of the patients with a mean of 5.76 ± 1.66 mg/dl. The mean age was 48 ± 14.37 years, BMI 24.98 ± 6.09 kg/m 2 , URR 71.88 ± 8.52%, and Kt/v 1.32 ± 1.09.The main barrier to exercise was identified to be fatigue on HD days by 58.5% of patients, followed by fear of getting hurt (36.6%). Finally, 80.4% of patients were mildly malnourished and QOL scale was 65.02% ± 18.54. Conclusion. Our study highlighted the widespread of hyperphosphatemia and malnutrition in our sample. The IDE regimen, if proven effective in future studies, could be integrated in the routine practice and may improve patients'outcomes.
