Abstract: The indications for rigid bronchoscopy for interventional pulmonology have increased and include stent placements and transbronchial cryobiopsy procedures. The shared airway between anesthesiologist and pulmonologist and the open airway system, requiring specific ventilation techniques such as jet ventilation, need a good understanding of the procedure to reduce potentially harmful complications. Appropriate adjustment of the ventilator settings including pause pressure and peak inspiratory pressure reduces th… Show more

“…This technique is less popular in adults, as improved surgical conditions can be provided with muscle relaxants, which prevent coughing and reduce the risk of airway injury [1,3]. It is possible to deliver IPPV through the bronchoscope via a side port, but the application of positive pressure could cause a foreign body to move more distally in the bronchus, potentially causing distal air trapping or segmental collapse [4]. An alternative is to insufflate O 2 via the bronchoscope in an apnoeic technique, and only deliver IPPV intermittently [3].…”

“…The injector is attached to the side port of the bronchoscope, and intermittent ventilation given by manual device activation. Jet ventilation carries a risk of barotrauma [4], a key concern in this extremely elderly patient. Chest wall movement can be observed, but ET CO2 cannot be monitored.…”
