ISPD guidelines for peritoneal dialysis in acute kidney injury: 2020 update (adults)

Abstract: Summary statements (1) Peritoneal dialysis (PD) should be considered a suitable modality for treatment of acute kidney injury (AKI) in all settings (1B). Guideline 2: Access and fluid delivery for acute PD in adults (2.1) Flexible peritoneal catheters should be used where resources and expertise exist (1B) (optimal). (2.2) Rigid catheters and improvised catheters using nasogastric tubes and other cavity drainage catheters may be used in resource-poor environments where they may still be life-saving (1C) (minim… Show more

“…Patients have been treated with standard manual exchanges with the frequency of exchanges depending on the clinical condition of the patient, following ISPD guidelines for PD treatment of AKI. 3 Full renal function recovery was defined by the normalization of creatinine measured during the follow-up, 3 months after the discharge.…”

“…In this issue of Peritoneal Dialysis International (PDI) , Cullis et al have drafted the 2020 update to The International Society for Peritoneal Dialysis guidelines for PD in AKI in adults, with the paediatric guidelines soon to follow. 12 The guidelines are supported by an evidence base that includes randomized controlled trials, a Cochrane review and a wealth of global expertise particularly from colleagues in Brazil and Africa. 13 15 Eloquently providing the rationale and blueprint for implementation of PD for AKI, these guidelines will serve as a timely, invaluable and definitive clinical resource in the foreseeable future.…”

“…11 The ISPD guidelines on PD for AKI now recommend that PD fluid can be mixed using a combination of Ringers lactate or Hartmann's solution combined with 50% dextrose water, but only when commercially produced fluids are not available. 12 Unfortunately, in most low-resource countries in Africa it is very difficult to obtain commercial PD solutions due to both high costs and transport logistical challenges. As a result, the SYL programme recommends that if commercially PD fluid is not available, mixing PD solutions locally is a safe and effective alternative that can be utilised in low-resource areas.…”
