Objectives: This study aims to investigate the protective effect of hyperbaric oxygen therapy (HBOT) as an antioxidant agent on cisplatin ototoxicity. Materials and Methods: Between June 4 th , 2014 and June 13 th , 2014, a total of 15 adult female Wistar albino rats were used in this study. The rats were divided into the following groups: Group A comprised of five rats and had prophylactic one course of HBOT before intraperitoneal (IP) cisplatin injection, followed by two more courses of HBOT. Group B comprised of five rats receiving three cycles of HBOT after 10 mg/kg of IP cisplatin administration. Group C served as the control group, including five rats, and received 10 mg/kg IP cisplatin without any additional treatment. Neuropathic pain is a kind of chronic intractable disease. HBO explored the involvement of mitophagy which is associated with energy metabolism by AMPK. CaMKKβ/AMPK signaling pathway may involve in mitophagy process. We randomly divided mice into four groups: C, S, CCI, and CCI + HBO group. Pain-related behaviors were evaluated using mechanical withdrawal threshold and thermal withdrawal latency analysis. Western blot was employed to assess expression of CaMKKβ, pAMPK, AMPK, NIX, BNIP3, and Drp1. Spinal cord was observed under the electron microscope. Immunofluorescence changes in NeuN and CaMKKβ or AMPK were examined. AMPK inhibitor compound C (CC) and CaMKKβ inhibitor STO609 were administrated prior to CCI, respectively. The changes in behaviors and Western blot are examined. HBO upregulated CaMKKβ and pAMPKα expression. NeuN and CaMKKβ were colocalizated in immunofluorescence. HBO can elevate the pain-related behaviors significantly, while it was downregulated by CC or STO609. HBO upregulated NIX, BNIP3, and Drp1 expressive level more significantly than those in CCI group. However, expression was reduced when CC or STO609 were administered. Electron microscopic examination showed that mitophagy was upregulated after HBO treatment. This phenomenon was not observed when CC or STO609 were administered. Our findings suggest that CaMKKβ/AMPK pathway maybe a potential signal pathway on analgesic mechanism of hyperbaric oxygen via mitophagy.

Although the cochlear vascular supply (stria vascularis) is designed to block to certain compounds and molecules, it must enable gas exchange to survive. The inner ear capillaries must deliver oxygen and remove carbon dioxide for the cochlea to function. These gases diffuse through tissues across a concentration gradient to reach the desired target. Tight junctions or the endothelial basement membrane do not impede them. Therefore, gases that can diffuse into the inner ear are attractive as therapeutic agents. The two gases most often used in this way are oxygen and hydrogen, although carbon dioxide, ozone, and argon have also been investigated. Typically, oxygen is delivered as hyperbaric oxygen (HBO) (oxygen at pressure higher than atmospheric) to provide increased oxygen levels to the inner ear. This not only relieves hypoxia, but also has anti-inflammatory and other biochemical effects. HBO is used clinically to treat idiopathic sudden sensorineural hearing loss, and both animal and human studies suggest it may also assist recovery after acute acoustic trauma. Laboratory studies suggest hydrogen works as a free radical scavenger and reduces the strong oxidants hydroxyl radicals and peroxynitrite. It also has anti-apoptotic effects. Because of its anti-oxidant and anti-inflammatory effects, it has been studied as a treatment for ototoxicity and shows benefit in an animal model of cisplatinum toxicity. Gas diffusion offers an effective way to provide therapy to the inner ear, particularly since some gases (oxygen, hydrogen, carbon dioxide, ozone, argon) have important therapeutic effects for minimizing cochlear damage.

Abstract: HBO was found effective for prevention of cisplatin-induced ototoxicity in rats. Our study differs from other studies regarding using a promising treatment, which does not expose subjects to extra stress.
