Basal ganglia infarction in young children, mostly after mild head trauma, has been repeatedly reported. The pathogenesis and the risk factors are not fully understood. Lenticulostriate vasculopathy, usually referred to as basal ganglia calcification, is discussed as one of them. We describe five young (7-13 months old on presentation) male children who suffered from hemiparesis due to ischemic stroke of the basal ganglia, four of them after minor head trauma. All of them had calcification in the basal ganglia visible on computed tomography or cranial ultrasound but not on magnetic resonance imaging. Follow-up care was remarkable for recurrent infarction in three patients. One patient had a second symptomatic stroke on the contralateral side, and two patients showed new asymptomatic infarctions in the contralateral basal ganglia on imaging. In view of the scant literature, this clinic-radiologic entity seems under recognized. We review the published cases and hypothesize that male sex and iron deficiency anemia are risk factors for basal ganglia stroke after minor trauma in the context of basal ganglia calcification in infants. We suggest to perform appropriate targeted neuroimaging in case of infantile basal ganglia stroke, and to consider prophylactic medical treatment, although its value in this context is not proven.

The prevalence of iron deficiency anemia (IDA) in Indonesian under-five children is still high. Unhealty eating hab it of under-five children will result in reducing nutrition intake, e.g. iron and vitamin C. This study was aimed to analyze the contrib ution of micronutrient intake (iron and vitamin C) to IDA status of under-five children. A total of 185 under-five children in range of 12-59 months old who had the data of consumption (SKMI 2014) and b lood b iochemical (Riskesdas 2013) was used in this study. R esult showed that there were no significant differences in the level of adequacy of iron and vitamin C intake b etween all age groups (P > 0.05). There was a positive correlation significantly b etween the level of adequacy of iron intake in the normal category (≥77% RDA) with haemoglob in concentration (P < 0.05; r ˂ 0.5). There was a correlation b etween the level of adequacy of iron and vitamin C intake in the deficiency category (˂77% RDA) with ferritin and transferrin concentrations (P < 0.05; r ˂ 0.5). In conclusion the age of the young group (12-35 months) with the level of adequacy of iron intake ˂77% RDA is at risk of developing AGB. Kebiasaan makan balita yang kurang beragam menjadi salah satu faktor rendahnya asupan zat gizi, khususnya zat gizi mikro seperti zat besi. Penelitian ini bertujuan menganalisis kontribus i asupan zat besi dan vitamin C terhadap status AGB pada balita. Penelitian ini menggunakan data 185 anak balita dengan kisaran umur 12 -59 bulan yang memiliki data asupan makanan (SKMI 2014) dan biokimia darah (Riskesdas 2013). Hasil penelitian menunjukkan bahwa tidak ada perbedaan tingkat kecukupan asupan zat besi dan vitamin C pada tiap kelompok umur (P > 0,05). Terdapat korelasi positif antara tingkat kecukupan asupan zat besi pada kategori normal (≥77% AKG) dengan konsentrasi hemoglobin (P < 0,05; r ˂ 0,5). Terdapat korelasi antara tingkat kecukupan asupan zat besi dan vitamin C pada kategori defisiensi (˂77% AKG) dengan konsentrasi feritin dan transferin (P< 0,05; r ˂ 0,5). Dapat disimpulkan bahwa umur kelompok muda (12-35 bulan) dengan tingkat kecukupan asupan zat besi ˂77% AKG beresiko mengalami AGB. METHODSThe integrated HTA approach shall include causal chain analysis (CCA), systematic literature review (SLR) and economic modelling. Causal Chain Analysis-Causal chains for anaemia in the children shall be assembled consisting of system characterisation, identification of the activities and the impacts, and their links to the anaemia impacts. Systematic Literature Review-The factors identified by the CCA will define the scope of the SLR and additional factors will be further explored. RCTs and SLRs which evaluated the effect of interventions for childhood anaemia shall be searched and included. An approach proposed by the Cochrane Collaboration and GRADE profiler shall be used. Economic Modelling-Economic model shall estimate the costs/benefits/drawbacks related with childhood anaemia in the developing countries and ensuing influence on the health-care system. The structure will be determined based on available research evidence. The data will be obtained from SLR or other pertinent evidences. RESULTSResults of the Systematic Literature Review, the Causal Chain Analysis, and the Economic Modelling will be combined together to create a consolidated report. CONCLUSIONSHTA shall be useful to policy-makers and health-care personnel in developing completely novel ways of understanding how different intervention effects change in childhood anaemia in developing countries.
