Ipilimumab-Induced Sarcoidosis in a Patient With Metastatic Melanoma Undergoing Complete Remission

Abstract: A 49-year-old patient was diagnosed in 1996 with a melanoma on the right calf (nodular malignant melanoma; Breslow thickness, 2.0 mm; Clark level, 4), for which he underwent local resection elsewhere.Half a year later, he developed pathologically enlarged lymph nodes in the right groin, and a superficial and deep node dissection was performed (31 of 40 nodes were positive for melanoma), followed by irradiation of the inguinal and iliac regions (45 Gy in nine fractions). After the lymph node dissection and irra… Show more

“…A positive finding on PET imaging in patients with melanoma treated with Ipilimumab can represent other entities including iatrogenic sequelae from immunotherapy [11]. With the emergence of novel immunotherapy treatment for metastatic melanoma and an increase in the popularity of PET imaging as a modality for diagnosing metastatic disease this article highlights two important issues; the traditional role of biopsies in confirming metastatic disease and immune related adverse events associated with novel therapies [1].…”

“…Previous case reports [9,10,11,17,18] have documented the development of sarcoidosis in patients being treated with ipiliumumab. However, to our knowledge, the development of neurosarcoidosis has not been previously reported as a complication.…”

“…For example, cases of sarcoid-like reactions have been reported following treatment of melanoma with interferon, vemurafenib, vaccine therapy, ipilimumab and nivolumab [9][10][11][12][13][14]. In our institutional experience, we have seen patients with concurrent melanoma and sarcoidosis both identified at the diagnosis of malignancy, and the development of sarcoid-like reactions, both symptomatic and asymptomatic from ICPI therapy [15].…”
