Iodine-131—Metaiodobenzylguanidine Double Infusion With Autologous Stem-Cell Rescue for Neuroblastoma: A New Approaches to Neuroblastoma Therapy Phase I Study

Abstract: A B S T R A C T PurposeIodine-131-metaiodobenzylguanidine ( 131 I-MIBG) provides targeted radiotherapy with more than 30% response rate in refractory neuroblastoma, but activity infused is limited by radiation safety and hematologic toxicity. The goal was to determine the maximum-tolerated dose of 131 I-MIBG in two consecutive infusions at a 2-week interval, supported by autologous stem-cell rescue (ASCR) 2 weeks after the second dose. Patients and MethodsThe 131 I-MIBG dose was escalated using a 3 ϩ 3 phase I… Show more

“…177 Lu-DOTATATE offers a unique treatment target and differs from 131 I-MIBG and chemotherapy in its mode of action. We recognize the success of 131 I-MIBG therapy (26)(27)(28)(29) and do not propose 177 Lu-DOTATATE as a competitor but as a therapeutic option for those who have relapsed or have primary refractory disease and who may or may not have already received 131 I-MIBG therapy. A combination of the 2 agents may be more effective than either used alone.…”

“…This point is evident in our current and prior reports on salvage chemotherapy for NB [8][9][10][11] as well as in experience with 131 I-MIBG therapy. 39 PBSC support also has allowed the investigation of novel submyeloablative induction chemotherapy regimens against NB 37,40 and other solid tumors in children. 36,41 We conclude that, in patients with high-risk NB, turning to HD-ICE for salvage or consolidative therapy is an attractive option because of noncross-resistance with widely used retrieval regimens.…”
