Invasive mucormycosis during treatment for acute lymphoblastic leukaemia—successful management of two life-threatening diseases

Abstract: A 5-year-old patient treated for acute lymphoblastic leukaemia (ALL) developed proven pulmonary invasive fungal disease (IFD) due to Actinomucor elegans. While completing ALL treatment according to AIEOP ALL protocol 2009 for further 15 months, antifungal treatment with liposomal amphotericin B and intermittent additional posaconazole was continued until immune reconstitution 7 months after the end of ALL treatment. Repeated imaging guided treatment decisions. Twenty-six and 19 months after the end of ALL trea… Show more

“…A recent retrospective study pointed out that the combination of liposomal AmB together with posaconazole produced a significant synergistic effect for ensuring short-term survival in patients with hematologic malignancy and was more effective than monotherapy [ 435 ]. Similar results were seen in a patient with acute lymphoblastic leukemia, although the drug of choice in this case was AmB lipid complex [ 436 ]. This evidence shows the potential synergistic interaction that can be obtained with AmB in lipid formulation.…”

“…This is the first reported case of rhino-orbito-cerebral mucormycosis due to Actinomucor elegans in a pediatric patient with leukemia. It was previously reported in a 5-year-old patient with acute lymphoblastic leukemia, however, in this case an invasive pulmonary mucormycosis was developed [20].…”

“…One of the most important characteristics of Mucorales is their marked vascular tropism, which facilitates hematogenous dissemination and vascular thrombosis, causing lesions with extensive tissue necrosis 7 . This scenario hinders the arrival of leukocytes to the site of infection and the penetration of drugs into the tissues, complicating treatment 8 .…”
