Sudden Sensorineural hearing loss (SSNHL) is a medical emergency inwhich etiopathogenesis and consensus on treatment are still largely debatable. SSNHL has been defined as 30 dB or more sensorineural hearing loss over at least three contiguous audiometric frequencies occurring within 3 days or less. Although several vascular, inflammatory, viral and neoplastic causes exist, no specific etiology can be confirmed; thereforeit is considered idiopathic. Steroids form the mainstay of treatment; however, recent experience with transtympanic steroid injections has been very encouraging. The exact number of injections, dosage, steroid form and window of application is still to be determined conclusively. We report a rare case of idiopathic SSNHL with profound hearing loss treated by 6 shots of transtympanic injections which normalised the hearing. We also review the methods and improvement of transtympanic injections from other studies.

Objective: To evaluate the efficacy of intratympanic steroid treatment with concurrent systemic steroids as an initial therapy for sudden sensorineural hearing loss (SSNHL). Methods:Patients who were diagnosed as idiopathic SSNHL and received no therapy were included in the study. Patients were divided into control and study groups. Patients in the study group received concurrent intratympanic steroid treatment with systemic steroid therapy. Patients in the control group received standard systemic steroid treatment alone. The duration of time before the treatment was evaluated for all the patients. Pure tone audiometry (PTA) was performed for all the patients in the pre-treatment and post-treatment period in order to record the responses for both in terms of the individual frequencies and the pure tone average. Results:Twenty nine patients (16 female and 13 male) with a mean age of 43.6 years were enrolled in the study.There was 15 patients in the study group and 14 patients in the control group. The mean duration of time for the initial therapy was 4 days for the control group and 6.5 days for the study group (p=0.125). The mean recovery in PTA for the control group was 10.29±4.10 decibel (dB) (0-15) while it was 19.40±18.84 dB (3-68) for the study group (p=0.003). Conclusion:At the end of treatment, significantly better hearing results were obtained for the patients in the study group when compared to the control group both for PTA and all the frequencies individually. We suggest that concurrent intratympanic steroids with systemic steroid therapy gives better hearing results than the standard systemic therapy alone.

Background: To assess the clinical and audiometric profile in patients presenting with sudden sensorineural hearing loss (SSNHL).The study also aimed to find out any possible etiology of SSNHL, prognostic factors and effectiveness of treatment. Prospective, open label, randomized study conducted in department of ENT at the Ram Lal eye and ENT hospital attached to Government medical college, Amritsar, Punjab, India.Methods: 50 patients of either sex aged between 18 to 50 years suffering from unilateral or bilateral onset SSNHL included in the study on the basis of predetermined clinical criteria. The hearing assessments of patients were done before and after the treatment with tuning fork tests, pure-tone and impedance audiometry. Results: Most of the patients of SSNHL presented with unknown etiology. Steroid treatment gave a statistically significant improvement in pure tone averages. Maximum improvement is observed in patients who started treatment within one month of developing SSNHL. Post-treatment outcomes were not influenced by age of the patient and type of the pure tone audiometry graph. The presence of vertigo and diabetes are bad prognostic factors for hearing outcome in SSNHL.Conclusions: Most of the cases of sudden sensorineural hearing loss have unknown etiology. Patients with associated DM and vertigo are having poor recovery of hearing. Early starting of treatment gives maximum improvement of hearing.
