Intramedullary nails for extracapsular hip fractures in adults

Abstract: Analysis 1.1. Comparison 1 Proximal femoral nail (PFN) versus Gamma nail, Outcome 1 Mortality.. .. .. . Analysis 1.2. Comparison 1 Proximal femoral nail (PFN) versus Gamma nail, Outcome 2 Final functional outcomes. Analysis 1.3. Comparison 1 Proximal femoral nail (PFN) versus Gamma nail, Outcome 3 Harris hip scores (0 to 100: high values = best function

“…Apart from these cases, standard therapy is dependent on the type of fracture and includes osteosynthesis using dynamic hip screws (DHS), intramedullary nails (e.g., PFN), or alloarthroplasty [2][3][4]. There is a large body of evidence from studies that consistently show high morbidity and mortality in association with this injury, mainly in elderly patients [5,6].…”

“…Although use of sliding hip screws (SHSs) is still considered a gold standard in treatment of these fractures, there is a wide tendency in using cephalomedullary nails because of their biomechanical superiority over SHS. 1,2 Besides, a cephalomedullary nail can be inserted by a minimally invasive technique, as the closed reduction of the fracture preserves the haematoma and minimally invasive technique allows the surgeon to minimise soft-tissue dissection, thereby reducing surgical trauma, blood loss, infection and wound complications. 3,4 Starting from introduction of gamma nails, different designs of cephalomedullary nails have been used in these fractures to improve functional results and decrease the complications.…”

“…Hip fractures are increasing in numbers and most commonly occur in elderly women [1,2]. The mean age of a patient who suffers a proximal femur fracture has been estimated to be above 80 years.…”
