Background: Patent ductus arteriosus is a congenital condition with high morbidity, especially in preterm infants of extremely low birth weight, representing 5% to 10% of congenital heart diseases. Our objective was to describe the approaches used at a reference hospital for the percutaneous occlusion of PDA. Methods: We conducted a retrospective study on the transcatheter treatment of patent ductus arteriosus from April of 2008 to April of 2010. Results: Forty-seven cases were reviewed and most of them (78.8%) were treated with Flipper™ coils while the remaining patients received the Amplatzer™ device. Ductal morphological configuration was Krichenko type A in 89.4% (34 in the Flipper™ coil group and 8 in the Amplatzer™ group), type D in 6.4% (2 in the Flipper™ coil group and 1 in the Amplatzer™ group) and type E in 4.2% (1 in each group) of patients. Pre-catheterization minimum diameters were 2.6 ± 0.8 mm and 3.8 ± 1.6 mm for the Flipper™ coil and Amplatzer™ groups, respectively. Immediate total occlusion of the defect was obtained in the control angiography in 72.3% of the patients. Seven patients treated with the Flipper™ coil received additional coils and two patients treated with the Amplatzer™ device presented minimal residual shunts. There were no procedure-related complications. In the follow-up after hospital discharge, one patient presented minimal residual shunt at the echography, 45 days after catheterization. Conclusions: Percutaneous patent ductus arteriosus occlusion has proven to be safe and effective in most cases.

Background:The percutaneous approach is the therapy of choice in neonates with critical pulmonary stenosis (PS) and pulmonary atresia with intact interventricular septum (PAIVS). Consecutive cases treated at a reference center and their midterm follow-up was reported in this study. Methods: Most of the patients received an intravenous infusion of prostaglandin preoperatively, regardless of the need. For the PAIVS approach, a stiff-tip guidewire was used and more recently, radiofrequency valvotomy. In most cases, sequential balloon dilation was performed until 110% to 120% of the pulmonary valve ring diameter was reached. Results: Between 1998 and 2011, 17 neonates with PAIVS (median age of 5 days, mean weight of 3.1 + 0.6 kg) and 30 neonates with PS (median age of 12 days, mean weight of 3 + 1.4 kg) were treated at a tertiary center. In PAIVS patients, a stiff-tip guidewire was used in 14 cases, with a success rate of 71.4%, and radiofrequency perforation in 3 cases, with a success rate of 100%. In PS patients, success was achieved in all of the cases. Hospital death was 23.5% for the PAIVS group and 3.3% for the PS group, none related to the percutaneous procedure. In the midterm follow-up, the reintervention rate due to restenosis was 21.4% in the PAIVS group and 10% in the PS group. Conclusions: Pulmonary valvuloplasty in PS or PAIVS has acceptable clinical and hemodynamic results, as long as favorable anatomic characteristics are observed and patent pulmonary flow is maintained until the procedure. Morbidity and mortality of patients with critical PS are lower than patients with PAIVS.

Objective This article aims to present updated therapeutic options for fetal congenital heart diseases. Methods Data source for the present study was based on comprehensive literature retrieval on fetal cardiac interventions in terms of indications, technical approaches and clinical outcomes. Results About 5% of fetal congenital heart diseases are critical and timely intrauterine intervention may alleviate heart function. Candidates for fetal cardiac interventions are limited. These candidates may include critical aortic valve stenosis with evolving hypoplastic left heart syndrome, pulmonary atresia with an intact ventricular septum and evolving hypoplastic right heart syndrome, and hypoplastic left heart syndrome with an intact or highly restrictive atrial septum as well as fetal heart block. The advocated option are prenatal aortic valvuloplasty, pulmonary valvuloplasty, creation of atrial communication and fetal cardiac pacing. Conclusion Fetal cardiac interventions are feasible at midgestation with gradually improved technical success and fetal/postnatal survival due mainly to a well-trained multidisciplinary team, sophisticated equipment and better postnatal care.
