International consensus conference on stool banking for faecal microbiota transplantation in clinical practice

Abstract: Although faecal microbiota transplantation (FMT) has a well-established role in the treatment of recurrent Clostridioides difficile infection (CDI), its widespread dissemination is limited by several obstacles, including lack of dedicated centres, difficulties with donor recruitment and complexities related to regulation and safety monitoring. Given the considerable burden of CDI on global healthcare systems, FMT should be widely available to most centres.Stool banks may guarantee reliable, timely and equitabl… Show more

“…Despite these success rates, several adverse events have been reported post FMT, including aspiration pneumonia/ pneumonitis following upper FMT or during sedation for colonoscopy, infections including bacteremia, abdominal pain, nausea, and irritable bowel syndrome (IBS). A recent guideline was published to provide guidance for practitioners to ensure quality control measures while handling FMT to prevent such adverse events [13]. e clinical question in our case was the safety of administering FMT in severely immunocompromised patients.…”

“…Therefore, the role of FMT must be further addressed by randomized, double-blind, placebo-controlled studies. Recent international consensus of stool banking for FMT have involved experts from Europe, North America, and Australia who proposed consensus guideline statements regarding several issues in stool banking, including the selection of donors and screening objectives, collection and processing of stool samples, monitoring of outcomes, ethical issues, and the evolving role of FMT in Clostridioides difficile infection and other diseases encountered in daily clinical practice [86]. about effectiveness and safety profile [85].…”

“…Despite a few worldwide existing stool banks, there is no standard method for producing material for FMT, and there are a multitude of factors that can vary between institutions offering this therapy [23]. To date, safety concerns and acceptability are the main constraints of therapeutic uses of FMT [24]. There are technical and logistical issues in establishing such a non-standardized treatment into clinical practice with safely, and with proper governance [25].…”
