Inlay Butterfly Cartilage Tympanoplasty in Children

Abstract: Inlay butterfly cartilage tympanoplasty is a safe, efficient, time-saving, and easy technique of tympanoplasty in children. Anatomic results may be improved by associating a split-thickness skin graft and/or by trimming a tragal graft much larger than the size of the perforation.

“…Eavey (3) describes it in 9% (n=1), Ghanem et al (4) in 14% (n=14), Rourke et al [7] in 11% (n=4) and Couloignier et al (9) in 5% (n=3) of patients. It was treated with 1 week of topical 0.3% ciprofloxacin eye drops and resolved in all four patients by 3 months after surgery (9). During our follow-up time in this retrospective study 20% (n=5) was noted.…”

“…During our follow-up time in this retrospective study 20% (n=5) was noted. The butterfly edge diminishes graft lateralization and displacement caused by patient activities, especially in children, and provides an instant 'locking' of the graft (4,9). No packing is required to 'stabilize' the graft.…”

“…7 Studies by Gierek et al and Couloinger et al have proven that there is no significant hearing difference between the use of temporalis fascia and cartilage for myringoplasty. 25,26 The composite chondro-perichondrial -clip‖ myringoplasty using the tragal cartilage and transcanal approach has a lot of advantages such as its a minimally invasive surgery done transcanally with an endoscope, it can be done in less than 30 minutes duration, it has good postoperative results in the form of perforation closure and improvement in hearing and it is cost-effective as it can be done under local anaesthesia and there is reduced hospital stay. We have taken cases with less than 35 dB hearing loss for our study, as middle ear ossicular pathology is rare in this class.…”

“…The varied autografts used are skin, perichondrium, vein, temporalis fascia, dura and cartilage. [4][5][6] Temporalis fascia remains the most frequently used graft material. Cartilage was first used in middle ear surgery for ossicular chain reconstruction in 1958 by Jansen 7 and the popular techniques for cartilage tympanoplasty described by MirkoTos include inlay butterfly, perichondrium-cartilage island flap, cartilage shield technique and palisade technique.…”
