Initial Experience With Laparoscopic Transvesical Ureteral Reimplantation at the Children’s Hospital of Philadelphia

Abstract: Laparoscopic intravesical reimplantation is in its infancy and appears to have higher complication rates in young patients with small bladder capacity.

“…In addition, the carbon dioxide within the bladder has no effect on the body's physiology. We also shared the views of Yeung et al [7] and Kutikov et al [11] that the use of transurethral feeding tube as a suction catheter was effective in clearing up urine or blood inside the bladder to provide a good surgical field.…”

“…Not only are the intramural dissection and suturing difficult, but the limited space within the bladder also represents another. Bladder contraction during the operation can also disrupt the surgical field [11]. To overcome this problem, carbon dioxide pneumovesicum up to a limited pressure was introduced by Yeung et al [7].…”

“…This is where minimal invasive laparoscopic techniques come in. Both intra-and extravesical laparoscopic approaches have been described [4][5][6] . This type of surgery can be technically challenging, but since the introduction of robot-assisted laparoscopy especially the extravesical Lich-Gregoir detrusorraphy technique seems a feasible and reasonable treatment option [7][8][9][10][11][12][13] .…”

“…Alternatively, surgery carries anesthetic and surgical risk. Complications include ureteric obstruction from either subureteric injection or ureteric reimplantation, resulting in invasive testing, surgeries, or renal failure [18,19]. However, the persistent impact on QOL is difficult to determine.…”
