Infectious endocarditis from Streptococcus bovis associated with colonic carcinoma: case report and literature review

Abstract: -Background -Many studies in the literature have warned of the need for investigation of colonic lesions among patients, especially elderly ones, who have bacteremia and/or endocarditis from Streptococcus bovis. Bacteremia and infectious endocarditis from Streptococcus bovis may be related to the presence of neoplastic lesions in the large intestine and hepatic disease. Aim -This report describes a patient who presented infectious endocarditis from Streptococcus bovis associated with colonic carcinoma and tubu… Show more

“…Moreover, infectious endocarditis from S. gallolyticus subsp. gallolyticus associated with colorectal cancer has been reported (15). It has been proposed that an increase in concentrations of cyclo-oxygenase2, interleukin 8 and cell proliferation might be involved in S. gallolyticus subsp.…”

“…Men tend to be more affected than women (1.7:1); however, the age of the affected individuals increased from 30-40 years in the pre-antibiotic era to 47-69 in the recent years. In developed countries, the degenerative valvular alterations and prostheses, and no longer rheumatic disease, are the most important predisposing factors for the occurrence of endocarditis 1,3,4 .…”

“…It is a Group D Streptococcus bacterium that can cause bacteremia and endocarditis, as well as urinary tract infection and septic arthritis, among others. It accounts for approximately 14% of infection causes and 13% of all cases of infectious endocarditis (IE) 1,2 . Sometimes the IE caused by S. bovis is associated with colorectal cancer.…”

“…Accordingly, the knowledge of association of colorectal adenoma with S. bovis/gallolyticus has important clinical implications. If the lesion can be discovered at an early stage, curative resection may become possible (Waisberg et al, 2002). Thus, bacteremia due to S. bovis/gallolyticus should prompt rigorous investigation to exclude both endocarditis and tumors of the large bowel (Beeching et al, 1985;Konda & Duffy, 2008).…”
