Cardiovascular complications are relevant due to their frequency and severity on the hypertension scenario. Studies refer Pharmaceutical Care (PC) as capable of decreasing cardiovascular risk rate (%CVR) on hypertensive patients. This study aimed to investigate, through a randomized clinical assay, the influence of PC service on the %CVR of hypertensive patients assisted in a health primary care unit from Fortaleza-Ceará. Two study groups were formed: i. Intervention Group (IG), which received orientation about taking medicines, actions aiming to prevent/solve medicine interactions and adverse effects and non-pharmacological interventions for 9 months and, ii. Control Group (CG), which received traditional assistance of the unit and was monitored during the same period. It was observed a statistically significant reduction on %CVR (10.76 to 7.86; p=0.04) and systolic blood pressure levels (SBP) (137.69 to 131.54; p<0.01) in the IG, while, in the CG, there was no significant alteration. 151 Drug Related Problem (DRP) were identified and it was realized 124 pharmaceutical interventions, with 89.2% of them resulting on solution/prevention of the problem. Our findings indicated that the inclusion of the PC service in the hypertensive health assistance was more effective at the %CVR and the SBP reduction in comparison to the traditional assistance offered.Uniterms: Pharmaceutical care. Cardiovascular risk. Hypertension.As complicações cardiovasculares apresentam relevância devido à sua freqüência e gravidade no contexto da hipertensão. Estudos referem que a prestação do Cuidado Farmacêutico (CF) é capaz de reduzir a taxa de risco cardiovascular (%RCV) em hipertensos. Esse trabalho objetivou investigar, com um ensaio clínico randomizado, a influência da prestação do CF na %RCV em hipertensos atendidos em uma unidade de atenção primária à saúde de Fortaleza-Ceará. Formarm-se dois grupos de estudo: i. Grupo Intervenção (GI), que recebeu orientações sobre tomada dos medicamentos, ações visando prevenir/ resolver interações medicamentosas e reações adversas e intervenções não-farmacológicas por 9 meses e ii. Grupo Controle (GC), que recebeu assistência tradicional da unidade e foi monitorado durante o mesmo período. Observou-se redução estatisticamente significativa nas %RCV (10,76 to 7,86; p=0,04) e nos níveis de pressão arterial sistólica (PAS) (137,69 to 131,54; p<0,01) no GI, enquanto no GC, não houve alteração significativa. Identificaram-se 151 Problemas Relacionados com Medicamentos (PRM) e foram realizadas 124 intervenções farmacêuticas, das quais, 89,2% resultaram em solução/prevenção dos problemas. Nossos achados indicaram que a inclusão do serviço de CF na assistência ao paciente hipertenso foi mais eficaz na redução da %RCV e níveis de PAS em comparação à assistência tradicional.

The specialized component for pharmaceutical services (Componente Especializado da Assistência Farmacêutica -CEAF) consists of a strategy to access high-cost treatments and new diseases that seeks to ensure full pharmaceutical treatment at the outpatient level. The aim of this study was to carry out a pharmacotherapy review in patients attending the CEAF program in a county at Santa Catarina, Brazil. This was a cross-sectional study through interviews conducted from August through September 2014. Twenty-two patients (59.9% men; mean age 47.9 years; SD 15.2) were interviewed. Among them, 42.9% had completed high school, and 33.3% had completed primary school. The average number of drugs per patient was 4.7 (SD 3.5), of which 30.1% were for the cardiovascular system and 26.2% for the nervous system. The most prevalent disease was schizophrenia (23.1%). Among the participants, 54.6% had problems with the use of medications, such as dose schedule, interaction between medicines and interaction with food, and 90.9% had problems related to pharmaceutical therapy, such as medication storage and transport. These patients received oral instructions and review reports with written guidelines, schedules, and pictograms. The higher the number of drugs used, the more frequent the problems were. The participants reported that the pharmaceutical services helped them to take their medications correctly. This study highlights the importance of the specialized pharmaceutical services to achieve better therapeutic results, considering the high cost of treatments offered by the CEAF program.Keywords: Pharmaceutical services; patient safety; Adherence to medication. RESUMOO CEAF (Componente Especializado da Assistência Farmacêutica) consiste em uma estratégia de acesso a tratamentos de alto custo e novas doenças que busca garantir a integralidade do tratamento medicamentoso em nível ambulatorial. O objetivo do estudo foi realizar a Revisão da Farmacoterapia em pacientes atendidos pelo programa CEAF em um município do Sul Catarinense. Tratou-se de um estudo transversal por meio de entrevistas realizadas entre agosto e setembro de 2014. Foram 22 pacientes com idade média de 47,9 (SD 15,2) anos, dos quais 59,9% eram homens. Do total de entrevistados, 42,9% tinham o ensino médio completo e 33,3%, nível fundamental. A média de medicamentos por paciente foi 4,7 (SD 3,5), sendo 30,1% para o sistema cardiovascular e 26,2% para o sistema nervoso. A doença mais prevalente foi a esquizofrenia (23,1%). Use of therapeutic outcomes monitoring method for performing of pharmaceutical care in oncology patients

This study aimed to implement pharmaceutical care using the therapeutic outcome monitoring (TOM) method for pharmacotherapeutic follow-up of oncological patients. This was a prospective longitudinal study involving patients undergoing oral chemotherapy. The study environment was an outpatient pharmacy at a tertiary-level oncology hospital. Ninety patients who received oral chemotherapy were evaluated, and 27 patients were followed up in accordance with the exclusion criteria and acceptability of participation in the study. The patients were predominantly diagnosed with gynecological tumors, with a mean age of 57.56 ± 13.06. The average consumption of drugs per patient was 4.63 ± 4.85, and more than 55% of patients had undergone oral antineoplastic therapy for more than a year. The main therapeutic groups used were drugs that acted on the gastrointestinal tract and metabolism (34%). All patients had at least one drug-related problem (DRP). In total, 133 DRP were identified. Approximately 33% of patients had DRPs related to antineoplastic therapy; non-adherence, incorrect administration, and the probability of adverse events were among the frequently reported DRPs. We identified 43 negative outcomes associated with medication (NOM), with untreated health problems (47%) and non-quantitative insecurity (30%) being the most frequently reported. 81 pharmaceutical interventions were performed, and 96% were accepted. The main errors avoided with the interventions were untreated health problems, misuse, and interruptions associated with medication administration. The TOM method effectively achieved the desired results of therapy, improving the use of medicines, and thus increasing patient safety.
