Inased (inhaled sedation in ICU) trial protocol: a multicentre randomised open-label trial

Abstract: IntroductionThe use of sedation in intensive care units (ICUs) is necessary and ubiquitous. The impact of sedation strategy on outcome, particularly when delivered early after initiation of mechanical ventilation, is unknown. Evidence is increasing that volatile anaesthetic agents could be associated with better outcome. Their use in delirium prevention is unknown. Methods and analysisThis study is an investigator-initiated, prospective, multicentre, two-arm, randomised, control, open-trial comparing inhaled se… Show more

“…However, consistent with previous findings, the need for intravenous sedatives was decreased when inhaled sedation was used. In addition, the risk of developing delirium under inhaled sedation compared with intravenous sedation was not specifically analyzed in our cohort and warrants further investigation [38]. Fifth, the patients in our cohort mainly received sevoflurane through the Sedaconda-ACD (Sedana Medical, Danderyd, Sweden), and whether the choice of other volatile anesthetics and/or device used for ICU sedation could impact clinical outcome remains undetermined.…”

“…Cognitive function is an important patient-centred outcome and up to 80% of ventilated adult ICU patients experience delirium in the hospital [ 26 ▪▪ ], with many experiencing longer lasting neurocognitive changes [ 27 ]. Sedation plays a vital role in neurological recovery and trials are underway to study the effect of inhaled anaesthetics on ICU delirium [ 28 ]. Although volatile anaesthetics are relatively inexpensive, there are considerable differences in equipment needed to administer intravenous and inhaled sedation.…”
