Improved survival and 30-day mortality after gastrostomy in Scottish motor neurone disease patients: evidence from a national retrospective cohort study using STROBE criteria

Abstract: Objectives: Defining historical changes and outcomes in the use of gastrostomy in the management of Scottish MND patients. Methods: The 1989-1998 and 2015-2016 Scottish national MND cohorts were used to examine the frequency, timing, and survival related to gastrostomy. The cohorts were censored for survival analysis. Results: There were 261 cases, 119 (46%) from the new register (2015-2016) and 142 (54%) from the old register (1989-1999). Percutaneous endoscopic gastrostomy (PEG) tubes were used exclusively i… Show more

“…Implementation of life extending treatment-gastrostomy Enteral feeding-affects quality of life rather than survival and occurs in 10-20% of patients. Gastrostomy extends life by about 3 months based on data in Gorrie et al [23]. As the trial is taking a treatment policy perspective, this event will be handled by using a treatment policy strategy P. Implementation of life extending treatment-ventilation Trachaeostomy extends life but is very rare.…”

Implementation of life extending treatment—ventilation Trachaeostomy extends life but is very rare. Non-invasive ventilation (NIV) via a mask is more common, occurring in 5% of patients soon after diagnosis and overall 10–20% would be expected over 18 months.…”

“…Gastrostomy tube insertion is recommended by several organizations, including the American Academy of Neurology, 3 the European Federation of Neurological Societies, 4 and the UK National Institute for Health and Care Excellence 5 . Gastrostomy placement may provide a survival benefit in addition to its overt symptomatic benefits, yet its survival benefit has been challenged 6–8 . Although percutaneous endoscopic gastrostomy had been a common technique for gastrostomy tube insertion, over the last decade, many institutions have been performing radiologically inserted gastrostomies (RIGs) 9,10 .…”
