Implementing trials of complex interventions in community settings: The USC–Rancho Los Amigos Pressure Ulcer Prevention Study (PUPS)

Abstract: Background Randomized trials of complex, non-pharmacologic interventions implemented in home and community settings, such as the University of Southern California (USC)–Rancho Los Amigos National Rehabilitation Center (RLANRC) Pressure Ulcer Prevention Study (PUPS), present unique challenges with respect to: (a) participant recruitment and retention, (b) intervention delivery and fidelity, (c) randomization and assessment, and (d) potential inadvertent treatment effects. Purpose We describe the methods emplo… Show more

“…A range of trials of interventions for PUs appears in the published literature, specifically in people with SCI, some of which may not be captured in existing systematic reviews. Preventative interventions include those which involve self-efficacy enhancement [87] and lifestyle management [88,89], whilst interventions for treatment include irradiation with ultraviolet-C [90], telephone-based support [91] and negative pressure wound therapy [92]. These are clearly candidates for future consideration in systematic reviews and updates.…”

“…The intervention framework is based on principles of Lifestyle Redesign, a preventive occupational therapy intervention originally developed to address the needs of community dwelling, ethnically diverse older adults (17, 18), which has since been adapted to address health-promoting lifestyle change in a variety of populations (19, 20). The Lifestyle Redesign intervention framework has been shown in two large-scale randomized controlled trials (RCTs) to cost-effectively improve physical and mental well-being, and facilitate maintenance of therapeutic gains over the long term (17, 18, 21, 22).…”
