Impact of Proximal Colon Retroflexion on Adenoma Miss Rates

Abstract: A second examination by retroflexion in the proximal colon did not increase the calculated miss rate relative to that performed by a forward view examination. These results do not support the addition of routine right colon retroflexion to colonoscopy.

“…New colonoscope techniques have been used to help reduce the missing rate. Wide-angle (170°) colonoscope was expected to abate blind zone during colonoscopy procedure, but this was still limited in reducing polyp missing rate (26). High definition colonoscopy (27), Narrow-band imaging (NBI) (8,28,29), and Autofluorescence imaging (AFI) (30,31) have been carried out to highlight colorectal polyps from normal mucosa to raise polyp detection rate.…”

“…Both adult and standard colonoscopes can be placed into retroflexion in the proximal ascending colon and are withdrawn in retroflexion to the hepatic flexure. There is currently no evidence that a second examination performed in retroflexion is superior to a second examination performed in the forward view [65]. In a series of 1000 consecutive patients undergoing primarily screening and surveillance colonoscopy, a second examination in retroflexion after removal of all lesions detected in the forward view identified a miss rate for the first examination of 9% for all polyps, and five lesions 1 cm or larger were identified in retroflexion [64].…”
