Impact of piperacillin-tazobactam shortage on meropenem use: implications for antimicrobial stewardship programs

Abstract: Drug shortages pose a clear detriment to antimicrobial stewardship (AS) efforts. Our objective was to evaluate the effect of a piperacillin-tazobactam shortage on meropenem use, related costs, and associated changes in AS activity. A quasi-experimental quality improvement review compared adult patients receiving meropenem ≥72h three months pre-shortage and three months during the shortage. 320 patients were included (pre-shortage: 103; shortage: 217). Baseline characteristics were similar, but the length of st… Show more

“…Of 58 studies reporting mortality rate, 57 identified 73 relevant outcomes related to mortality rate. Among those studies, 54 studies reported 58 all-cause mortality rate outcomes [7, 11, 15, 1922, 27, 28, 3133, 35, 38, 40, 4448, 5052, 59, 62, 70, 72, 73, 7582, 85, 91, 92, 95, 96, 98, 99, 102, 104, 105, 109, 110, 112, 113, 116119]. Additionally, only 11 of the 57 relevant studies reported 15 infection-related mortality outcomes [7, 20, 48, 57, 74, 78, 79, 81, 83, 99, 109].…”

“…Of 91 studies, 87 measured 94 pertinent antimicrobial expenditure outcomes but only 46% of outcomes were assessed for statistical significance [8, 9, 1222, 26, 28, 3034, 37, 3948, 52, 53, 55–58, 60, 6268, 7073, 75, 76, 8284, 8789, 91, 93, 95, 97, 99, 100, 104, 106, 107, 117, 118, 120, 122138]. Most studies were conducted in hospitals with 500–1000 patients (26%).…”

“…The effect of drug shortages have an impact on, for example, the shortage of piperacillin-tazobactam on meropenem consumption which have a subsequent effect on antimicrobial stewardship activity. ASP should closely monitor shortages and establish hands-on strategies to prevent improper consumption of antimicrobials [ 7 ].…”

“…Differential diagnosis and the comorbidities of the critical ill subjects involved should be considered. It is important to modify antimicrobial therapy according to the sensitivity results in order to reduce costs, decrease the development of resistance and minimize the incidence of side effects and shortage [17,[19][20][21].…”
