Impact of Opioid Use in Hematological Malignancies: Clinical, Immunological and Concomitant Aspects

Abstract: Opioid agents play a unique role in pain and symptom management for cancer patients. Research shows that opiate use, especially when associated with underlying cancer, has significant effects on hematological parameters. These changes may lead to greater risk for immunosuppression, tumor growth and progression of metastatic processes. The aim of this review is to explore the effects of opiates on various metabolic and biological processes, as well as the hematopoietic system, especially in cancer patients. Our… Show more

“…Adequate postoperative pain relief during the early postoperative period seems to carry the greatest clinical implications for oncologic outcomes after pancreatic resection [ 4 , 5 ]. On the other hand, concerns have grown about unnecessary opioid use [ 8 ]. Nonetheless, opioids have been the mainstay of pain control after pancreas surgery.…”

“…At a molecular level, mounting evidence suggests that opioid‐induced immunosuppression is mediated by microRNA (miRNA). Morphine was shown to inhibit production of transcription factors while inducing miRNAs which are anti‐inflammatory, an effect seen in long‐term human models as well as murine studies 19 …”

“… 36 However, traditional opioids, such as morphine, fentanyl and sufentanil, may inhibit immune function, promote tumor vascular growth, promote tumor proliferation, promote tumor migration and simultaneously induce somatic-dependent risk. 37 , 38 Therefore, understanding how to reduce perioperative pain, protect immune function and reduce the risk of tumor recurrence and metastasis has become an important topic in the practice and research of anesthesia and perioperative management in cancer patients.
