ObjectivesSmart device apps for diabetes have the potential to support patients in their daily disease management. However, uncertainty exists regarding their suitability for empowering patients to improve self-management behaviors. This paper addresses a general research gap regarding theoretically based examinations of empowerment in diabetes research, by examining how diabetes app features correspond with conceptual indicators of empowerment.MethodsWe examined features of 121 apps for diabetes self-management available in Singapore, with the second highest proportion of diabetes among developed nations, for psychological empowerment (feeling of empowerment) and for behavioral empowerment (social support).ResultsDiabetes apps studied offered a narrow range of features, with limited feature-sets corresponding to indicators of empowerment. Customization as a strategy to improve perceived relevance of diabetes self-management as an indicator of psychological empowerment was especially limited. Moreover, there was a lack of features supporting patients' communication with healthcare professionals and within their private social networks.ConclusionsMobile apps for diabetes self-management failed to provide relevant features for empowering patients. Specific practical recommendations target improved adoption, sustained usage, and effectiveness of diabetes self-management apps.

Over the past 20 years, the Global Initiative for Asthma (GINA) has regularly published and annually updated a global strategy for asthma management and prevention that has formed the basis for many national guidelines. However, uptake of existing guidelines is poor. A major revision of the GINA report was published in 2014, and updated in 2015, reflecting an evolving understanding of heterogeneous airways disease, a broader evidence base, increasing interest in targeted treatment, and evidence about effective implementation approaches. During development of the report, the clinical utility of recommendations and strategies for their practical implementation were considered in parallel with the scientific evidence.This article provides a summary of key changes in the GINA report, and their rationale. The changes include a revised asthma definition; tools for assessing symptom control and risk factors for adverse outcomes; expanded indications for inhaled corticosteroid therapy; a framework for targeted treatment based on phenotype, modifiable risk factors, patient preference, and practical issues; optimisation of medication effectiveness by addressing inhaler technique and adherence; revised recommendations about written asthma action plans; diagnosis and initial treatment of the asthma−chronic obstructive pulmonary disease overlap syndrome; diagnosis in wheezing pre-school children; and updated strategies for adaptation and implementation of GINA recommendations.

The literature review provides up-to-date information about the clinical course of bronchial asthma (BA) in smokers with small airway diseases. Special attention is paid to the combination of bronchial asthma and chronic obstructive pulmonary disease (COPD), namely asthma-COPD overlap syndrome (ACOS). According to literature data, in case of small airway duseases exacerbations are more often and severe in smokers with BA and ACOS. Besides, disease prognosis worsens due to reduction in the efficacy of a baseline therapy. Keywords: bronchial asthma, small airway disease, smoking-related phenotype, asthma-COPD overlap (BA-COPD phenotype). В литературном обзоре представлены современные сведения об особенностях клинического течения бронхиальной астмы (БА) у курильщиков с поражением малых дыхательных путей (МДП). Особое внимание уделено сочетанию бронхиальной астмы и хронической обструктивной болезни лёгких (ХОБЛ; COPD) – синдрому перекрёста БА-ХОБЛ (СПБАХ, asthma-COPD overlap, ACO; фенотип БА-ХОБЛ). Согласно литературным данным, в случае поражения МДП у больных БА с фенотипом курильщика и при сочетании БА-ХОБЛ чаще возникают и тяжелее протекают обострения, ухудшается прогноз заболевания, в т.ч. из-за снижения эффективности базисной терапии. Ключевые слова: бронхиальная астма, поражение малых дыхательных путей, фенотип курильщика, asthma-COPD overlap (фенотип БА-ХОБЛ).
