Impact and Effectiveness of Dual Aspiration Technique in Stent-Assisted Mechanical Thrombectomy: Recent Improvements in Acute Stroke Management

Abstract: The dual aspiration technique with an additional intermediate guide catheter placed closed to the stent retriever leads to decreased procedure time in the anterior circulation. Even in cases with higher thrombus load and treated in DAT, clinical outcome improved.

“…The rate of reperfusion with an mTICI grade ≥ 2a with the RECO device was 91%, whereas the rate of reperfusion with an mTICI grade ≥ 2b was 87%, rates in accordance with values in the lit-erature, which vary from 66.7% to 94.8%. [16][17][18][19][20] Functional independence at 90 days was attained in 63% of patients in the RECO group, again within the range described in the most optimistic studies (53%-71%). No significant difference was found in the rate of recanalization or functional independence between the RECO and Solitaire groups, confirming the efficacy of the device.…”

“…In this situation, the proximal anchoring technique was tried with balloon inflation of the BGC located in the proximal ICA to push the aspiration catheter. With the large bore aspiration catheter at the face of the thrombus, dual aspiration was applied with a 50 mL syringe at the BGC and aspiration pump at the aspiration catheter 7) . If distal delivery of the aspiration catheter failed even with a proximal BGC anhttps://doi.org/10.3340/jkns.2021.0158 choring technique, a combined approach MT with a distal stent retriever anchoring technique was applied to navigate the aspiration catheter at the occlusion site.…”

“…After the 3 to 5 minutes of waiting after stent deployment to promote stent clot integration 32) , the aspiration catheter with suction and the stent retriever were slowly removed as a single unit. At this time, a dual aspiration technique was used at the BGC with manual aspiration 15) .…”
