Abstract: Gastrointestinal stromal tumor (GIST) is the most common mesenchymal malignancy of the gastrointestinal tract. This tumor is resistant to conventional chemotherapy and radiotherapy. Although surgery has been the only effective treatment for GIST to date, it is not enough to manage metastatic GIST. Imatinib mesylate, a KIT tyrosine kinase inhibitor, is an oral agent that has been found to have a dramatic antitumor effect on metastatic GIST with either wild-type or mutant KIT. Although imatinib mesylate has been… Show more

“…These observations raise questions on the importance of the particular cellular target for anti-tumor efficacy using target-specific drugs. Clinical data suggest that, unlike c-kit mutations in GIST [63][64][65][66] or bcr-abl in CML [67,68], mutated RAF alone does not seem to drive the malignant phenotype in cancer patients [59]. Hence, target expression like Raf kinase, VEGF, or VEGF-R might be more likely a dynamic process in the context of cellular stress reaction, for instance after cytotoxic chemotherapy [57], rather than a constant issue allowing predictive testing prior target-specific therapies [69,70].…”
